14. DESCRIPTION OF AMENDMENT/MODIFICATION (Organized by UCF section headings, including solicitation/contract subject matter where feasible.)
The purpose of Amendment 0002 to Solicitation Number 2018-N-67615 is to do the following:
a) Extend the receipt date for submission of proposals to April 10, 2018 at 10:00 AM EST.
b) Re-issue a revised copy of Solicitation No. 2018-N-67615 per the changes noted on pages 2-8 of this Amendment. See continuation pages. Changes to the amended solicitation document hereto attached are noted and highlighted in red.
c) Upload Questions and Answers hereto attached received in response to Solicitation 2018-N-67615
d) Offerors are required to acknowledge receipt of this Amendment with proposal submission. See Block 14 on page 1 of the Solicitation and Block 11 of this Amendment.
Except as provided herein, all terms and conditions of the document referenced in Item 9A or 10A, as heretofore changed, remains unchanged and in full force and effect.

SPECIAL NOTICE: Offerors are hereby placed on notice that there are major changes to Section L and Section M of the RFP. All changes are identified in “red.” Offerors are advised to re-read Section L and Section M in their entirety to ensure you have a full understanding of the changes. When submitting your proposal, please make sure you are responding in accordance with these change.
RFP CHANGES:
******OFFERORS ARE STRONGLY ENCOURAGED TO READ THE REVISED RFP IN ITS ENTIRETY AS WELL AS ALL ATTACHMENTS FOR THE RFP**** Review in its entirety the amended changes identified below. These changes are HIGHLIGHTED IN RED throughout the RFP and applicable attachments.
1. Section B, Clause B.9, Task Order Pricing is changed to correct the numbering of the clause.
2. Section I, FAR Clause 52.232.20, Limitation of Cost, has been removed from the RFP.
3. Section C, Clause C.4, Task Area 1B: Communications and Outreach has been updated as follows:
- Utilize software, tools, scripts and software modifications to automate the production of information product libraries;

11. Section L, Clause L.15 Technical Proposal Preparation Instructions-Volume-I, A. Basic Contract has been updated as follows:
- However, appendices shall only include the staffing plan table required under Task Order A and Task Order B Technical Subfactor 2, resumes, and requested literature (if any). Appendices do not count against the page limit. Resumes shall be limited to 3 pages per resume.
12. Section L, Clause L.15 Technical Proposal Preparation Instructions-Volume-I, A. Basic Contract, 1. Technical Subfactor 1 has been updated as follows:
· -Technical Subfactor 1 “There is no page limit for Attachment J4. Offerors may submit Attachment J4 as either an appendix to the Technical Volume or provide it under Section 2 of the Technical Volume. Appendices are not included in the page limit count.
13. Section L, Clause L.15 Technical Proposal Preparation Instructions-Volume-I, Basic Contract, Technical Subfactor 2 – Personnel (Small Business Set-Aside IDIQ and the Non-Set-Aside IDIQ) has been updated as follows:
Deleted Language (Basic Contract Level for the Small Business Set-Aside IDIQ and Non-Set-Aside IDIQ):
- The Offeror shall provide a Staffing Plan including:
· A Staffing Plan Table identifying the name, key staff flag, degree, expertise & qualifications, proposed labor category, area of support, and hours per week of each individual contractor it plans to employ to meet the requirements.
· Note that each individual identified on the staffing plan are individuals that the Offeror may reasonably expect to be available to work immediately upon the start of work for the awarded IDIQ. If the names of each individual contractor are not available at the time of proposal submission, the Offeror may list “To Be Determined” in the name column. The name, qualifications and degree are the only items that may remain unspecified on the Staffing Plan Table. See Staffing Plan Table Template.
· The identification of key staff in the Staffing Plan Table.
· The key staff, which are a subset of the staff shown in the Staffing Plan Table, are staff that the Offeror agrees will be available to work immediately upon the start of work for the awarded IDIQ
· For each key staff individual identified, the Offeror must provide a (1) resume, and (2) letter of commitment indicating the proposed staff has reached an agreement with the Offeror to work with the Offeror and (3) statement of the key staff individual’s experience meeting GIS and public health contractual requirements of CDC/ATSDR.
· The number of hours per week that each key staff individual will provide must be identified in the Staffing Plan Table. If the Offeror proposes a key staff at less than 40 hours a week, the Offeror must (1) fully justify the rationale for this decision, and (2) address the reasons why a key staff individual would support the work in only a limited manner.

“Identify company resources needed at the BASIC CONTRACT LEVEL to manage, support, and respond to a contract of this size and complexity. Task 1 – ATSDR GRASP Communications, Outreach and Partnership Development
1. Prepare, refine, and share communication products focused on educating the broad public health community regarding the efforts of GRASP and the importance of GIS to public health investigation and practice.
2. Prepare and maintain a GRASP Communications Plan outlining the broad communications strategy for GRASP and defining specific communications products which support that strategy.
3. Maintain the GRASP Fact Sheet Library, which contains information outlining GRASP GIS efforts in support of ATSDR public health projects.
4. Maintain the GRASP Feature Article Database, a database of internet and intranet articles for distribution via the GRASP Intranet site (grasp.cdc.gov) and GRASP Place & Health Internet site.
5. Maintain GRASP Presentation Library, a library of presentations provided by GRASP to audiences at CDC/ATSDR and in the broader public health community.
6. Support the development of partnerships with the CDC Foundation, academic institutions, and other potential collaborators in the broad public health community.
