B.2 Type of task order
This is a Firm Fixed Price Task Order with Cost Reimbursable Travel CLINS Travel CLIN amounts have already been pre-determined.
B.3 Non-Personal Services
(a) Personal services shall not be performed under this task order contract. Although the Government may provide sporadic or occasional instructions within the scope of the task order contract, the Contractor is responsible for control and supervision of its employees. If the Contractor (including its employees) believes any Government action or communication has been given that would create a personal services relationship between the Government and any Contractor employee, the Contractor shall promptly notify the Contracting Officer of this communication or action.
(b) The contractor shall comply with, and ensure their employees and subcontractors comply with, CDC Policy titled “Identification of Contractors' Employees and Safeguarding Government Information.” No Contractor employee shall hold him or herself out to be a Government employee, agent, or representative. No Contractor employee shall state orally or in writing at any time that he or she is acting on behalf of the Government. In all communications with third parties in connection with this contract, Contractor employees shall identify themselves as Contractor employees and specify the name of the company for which they work. . The contractor is limited to performing the services identified in the contract performance work statement and shall not interpret any communication with anyone as a permissible change in contract scope or as authorization to perform work not described in the contract. All contract changes will be incorporated by a modification signed by the Contracting Officer.
(c) The Contractor shall ensure that all of its employees and subcontractor employees working on this contract are informed of the substance of this clause. The Contractor agrees that this is a non-personal services contract; and that for all the purposes of the contract, the Contractor is not, nor shall it hold itself out to be an agent or partner of, or joint venture with, the Government. The Contractor shall notify its employees that they shall neither supervise nor accept supervision from Government employees. The substance of this clause shall be included in all subcontracts at any tier.
(d) Nothing in this clause shall limit the Government's rights in any way under any other provision of the contract, including those related to the Government's right to inspect and accept or reject the services performed under this contract.

Section C - Description/Specification/Work Statement Performance Work Statement (PWS) Title of Project
TASK ORDER A: GRASP Program Evaluation, Monitoring, and Communication (Small Business Set-Aside IDIQ)
C.1 Background and Need GRASP, the Geospatial Research, Analysis, and Services Program (Division of Toxicology and Human Health Sciences (DTHHS), ATSDR) is a program that provides ongoing GIS (geographic information system) science, analysis, technology and visualization (“geospatial SATV”) support to ATSDR, NCEH, OPHPR, and multiple other CIOs across CDC/ATSDR. The mission of GRASP is to provide leadership and expertise in the application of the concepts, methods, and tools of geography and geospatial information science to public health investigation and practice across CDC/ATSDR and among the public health community. GRASP objectives are to (1) acquire, manage, and prepare geospatial data for use by GRASP and GRASP partners; (2) analyze and examine the geospatial trends and patterns relevant to diverse factors associated with environmental health, infectious and chronic disease, and emergency preparedness & response; (3) geospatially enable CDC/ATSDR scientists and systems with data, consulting, technology, and training; (4) collaborate with scientists and researchers at the CDC/ATSDR and among our public health partners to address diverse public health issues; (5) contribute to a vibrant geospatial community among public health professionals; and (6) embrace, leverage and promote technology.
A programmatic responsibility of GRASP is to (1) monitor, evaluate and report upon GRASP operations, programs, and initiatives to ensure the success and improvement of GRASP science, technology, and visualization efforts, (2) prepare and share communication products focused on educating the broad public health community regarding the efforts of GRASP and the importance of GIS to public health investigation and practice, and (3) engage in outreach focused on building collaborative partnerships with CDC/ATSDR programs and organizations in the broader public health community.
C.2 Project Objective The objective of this Task Order is to acquire consultation and technical assistance to support GRASP to monitor, evaluate, and report upon the work of GRASP, (2) prepare communication and outreach products, and (3) engage in partnership development efforts. To accomplish the objectives of this Task Order, the contractor must provide consultation, assistance, guidance, and support. The expertise required will include communication and program evaluation.
C.3 Scope of Work The Contractor, as an independent organization and not an agent of the Government shall furnish all labor, travel, and other costs associated with the provision of on-site Contract staff to support the activities and provide the deliverables specified within the Task Order Technical Requirements. As specified in the Technical Requirements, the Contractor shall (1) monitor, evaluate and report upon GRASP operations, programs, and initiatives, (2) prepare and share communication products focused on educating the broad public health community regarding the efforts of GRASP and the importance of GIS to public health investigation and practice, and (3) engage in outreach focused on building collaborative partnerships with CDC/ATSDR programs and organizations in the broader public health community. Specifically, the Contractor shall provide the following:
C.4 Technical Requirements (Base Period and any Exercised Options) The Contractor shall be responsible for the completion of the following technical requirements:
Task 1 – ATSDR GRASP Communications, Outreach and Partnership Development
1. Prepare, refine, and share communication products focused on educating the broad public health community regarding the efforts of GRASP and the importance of GIS to public health investigation and practice.
2. Prepare and/or refine GRASP proposal documents submitted to NCEH-ATSDR, as well as other public health partners, for funding consideration.
3. Prepare and maintain a GRASP Communications Plan outlining the broad communications strategy for GRASP and defining specific communications products which support that strategy.
4. Maintain the GRASP Fact Sheet Library, which contains information outlining GRASP GIS efforts in support of ATSDR public health projects.
5. Maintain the GRASP Feature Article Database, a database of internet and intranet articles for distribution via the GRASP Intranet site (grasp.cdc.gov) and GRASP Place & Health Internet site.
6. Maintain GRASP Presentation Library, a library of presentations provided by GRASP to audiences at CDC/ATSDR and in the broader public health community.
7. Support the development of partnerships with the CDC Foundation, academic institutions, and other potential collaborators in the broad public health community.
Task 2 – GRASP Program Monitoring, Evaluation and Reporting
1. Prepare and maintain a GRASP Monitoring & Evaluation Plan outlining the broad monitoring and evaluation strategy for GRASP and specifying key metrics, indicators, and measures which must be captured to best understand the success and impact of the work of GRASP.
2. Maintain organizational logic models associated with GRASP and GRASP Units including the Geospatial Support Training and Resources Unit (GeoSTAR), Geospatial Informatics, Systems, and Applications Unit (GIS), Rapid Response and Emergency Preparedness Support Unit (RREPS), and Geospatial Epidemiology and Applied Research Unit (GEAR).
3. Prepare a GRASP Annual Report including metrics and information on GRASP operations, scientific contributions, and public health engagement.
4. Support the specification and definition of key GRASP organizational metrics which will be captured and reported by GRASP staff.
5. Prepare surveys and other evaluation tools to enable GRASP to collect information on the work that it does with partners at the CDC/ATSDR and beyond.
C.5 Reporting Schedule A. Kick-Off Meeting The Contractor shall attend a kick-off meeting between the Contractor’s project manager and CDC/ATSDR COR and other CDC/ATSDR GRASP program leadership at the designated CDC/ATSDR office in Atlanta, Georgia, within 2 weeks after award or option period effective date, if the option is exercised.
For the option periods, teleconference is sufficient for participants.
B. Quarterly Reporting The contractor shall provide a written quarterly report, containing at a minimum the following information:
· A brief description of ongoing tasks, completed tasks.
· Significant events and accomplishments regarding the project.
· Any current or anticipated problems and solutions proposed.
· Planned Activities during the upcoming reporting period.
· Thoughts, recommendations, etc. for the project based on progress to date; and
· Planned time off during the upcoming reporting period.
The report is due to the CDC/ATSDRD Contracting Officer’s Representative (COR) and GRASP Program Leadership electronically (MS Word or PDF) no later than the 10th calendar day of the month following the close of the quarter. Thus, reports will be due in April (Jan/Feb/March quarter), July (April/May/June quarter), October (July/Aug/Sept quarter), and January (Oct/Nov/Dec quarter).
C. Quarterly Conference Call The Contractor shall initiate a quarterly conference call with the CDC/ATSDR COR and GRASP Program Leadership, to be held no later than the 30th calendar day of the month following the close of the quarter. The purpose of this call is ongoing review of task order performance. Contractor shall e-mail the agenda to the COR at least five (5) work-days prior to the conference call.
The Contractor shall document the discussion from the conference call in the following quarterly report. Any required performance improvements must be documented/reported in the following quarterly reports.
C.6 Government Furnished Property The Government shall provide office space for the Contractor located at the following CDC/ATSDR facility.

Due to the nature of the work, CDC/ATSDR will provide IT equipment (i.e., desktop computer or a laptop computer), telephone, and other office equipment and supplies as needed.

C.8 Place of Performance The work is to be performed primarily on-site at the following CDC/ATSDR location.

C.9 Travel On the average, the Government anticipates 6 trips of 1-5 days each during the base year and option years, if exercised. Travel may be domestic and international destinations. Travel may be for the purposes of (1) attending a conference, (2) collaborating with a public health partner, and/or (3) supporting a public health in an extensive manner at an alternate work location. Contractor attendance at conferences is required to (1) support the government in the presentation of oral and poster presentations, and (2) support Government in meetings with CDC/ATSDR public health partners. Contractor attendance at collaborative meetings is required to (1) support the government in the presentation of oral and poster presentations, and (2) support the Government to update partners on GRASP projects and programmatic capabilities. Contractor is required to obtain approval from the Contracting Officer for all travel prior to incurring travel expenses.
Travel shall be in accordance with the Federal Travel Regulations (FTR).

C.11 Quality Assurance Surveillance Plan (QASP) The QASP provides a systematic surveillance method for the performance based services and describes the methodology by which the contractor’s performance will be monitored. Reference the GRASP IDIQ SOW for more information about the QASP.
The contractor is responsible for management and quality control actions necessary to meet the quality standards set forth in the GRASP IDIQ SOW and this Task Order. The below listed Performance Standards and Surveillance Methods shall be used by the COR in the technical administration of this QASP. The Government will evaluate the contractor’s performance at least annually, if not more frequently, in the Contractor’s Assessment Performance Reporting System (CPARS), www.cpars.gov Reference the GRASP IDIQ SOW contract for additional information about CPARS.

C. 12 Section 508 Compliance
By agreeing to the terms of the GRASP Geospatial Science, Analysis, Technology and Visualization IDIQ Contract, the offerer must comply with Section 508 standards for all deliverables in this task order. The offerer must complete an HHS/CDC checklist(s) as needed for each deliverable specified. The applicable Section 508 standards to this task order are:
