About this file
This is a Form SDVOB 100 Service-Disabled Veteran-Owned Business (SDVOB) Utilization Plan document issued by the State of New York that applies to the New York State Department of Transportation (NYSDOT) solicitation for road salt strategic reserves across Regions 1, 3, 5, 7, and 9. The form must be submitted with any bid or negotiated contract proposal prior to contract award. Bidders are required to provide a detailed description of supplies and services to be provided by each certified SDVOB subcontractor or supplier, including the subcontractor's name, address, telephone number, email address, federal identification number, and NYS OGS certification status. The utilization plan must specify the dollar value of subcontracts, supplies, and services along with intended performance dates for each contract component. Road salt reserves must be physically available on or before January 1, 2027, and all delivery is to be completed by March 31, 2027, with supplies dedicated exclusively to NYSDOT use across the specified regions.
If a contractor is unable to fully meet the SDVOB goal established for the contract, the bidder must submit good faith efforts documentation and request a waiver. Submission of the completed form constitutes the contractor's acknowledgement and agreement to comply with SDVOB requirements under NYS Executive Law, Article 17b. Failure to submit complete and accurate information may result in bid rejection, noncompliance findings, or contract suspension or termination. The form requires certification by the preparer with signature, printed name and title, email address, and telephone number, with subsequent agency review documenting approval status, contract award date, estimated completion date, contract amount obligated, and notices of deficiency or acceptance as applicable.
