About this file
This document is the Conflict of Interest Questionnaire (Form CIQ) required by the Texas Ethics Commission under Chapter 176 of the Local Government Code, applicable to vendors conducting business with the Hays Consolidated Independent School District in Texas. The form must be completed and filed with the District's records administrator no later than the seventh business day after a vendor begins contract discussions, submits a proposal, or becomes aware of any potential conflicts of interest with local government officers. The questionnaire addresses three primary disclosure requirements: whether any local government officer or their family members will receive taxable income from the vendor (excluding investment income) exceeding $2,500 over a 12-month period; whether the vendor has given gifts with an aggregate value exceeding $100 to any officer or family member within the same timeframe; and whether any family relationships exist between the vendor and local government officers. Vendors must also disclose any employment or business relationships with corporations or entities where local government officers serve as officers, directors, or hold ownership interests of one percent or more.
Failure to file the completed questionnaire or knowingly providing false information constitutes a misdemeanor offense under Texas Local Government Code Section 176.006. The form requires vendors to identify themselves, name the specific local government officer(s) involved, describe the nature of any business or employment relationships, and provide signatures with dates. Updates to previously filed questionnaires must be submitted within seven business days of discovering inaccuracies or incompleteness. This disclosure requirement applies to all vendors with business relationships meeting the statutory definition, including those engaged in contract negotiations or formal solicitation responses with the District.
