This Amendment provides the following information:
• Provides a list of Revisions to drawings and specifications
• Provides information on questions and items discussed at the Pre-Proposal meeting held on June 1st, 2022
• Provides responses to RFI’s received to date.
• Provides a list of attendees from the Pre-Proposal Meeting held on June 1st, 2022
• Updates the Wage Determination to OR 20220025 with 4 modifications, dated 20
May 2022.
This amendment also extends the proposal date. Offers are now due 6/30/2022, at 1600 hours local time.

REV-001
See Revised Civil Drawing C3.1. Connection of water services to existing water line has been consolidated from two services to one and connection is now to existing 10” C900 waterline instead of existing 2” as originally shown

PRE-PROPOSAL MEETING QUESTIONS AND ITEMS DISCUSSED
PPM-001
Is the Forest Service project currently permitted?
Response: The Forest Service is currently working with Deschutes County on plan review. The Forest Service will pay for building permit fees. The Contractor will be responsible for scheduling required building inspections. The contractor will be responsible for paying for any reinspection fees due to failed inspections.

PPM-002
Is a NPDES permit required for this project?
Response: No, a NPDES permit is not required however Contractor will be required to develop and submit an Erosion Control and Sediment that meets EPA requirements. See specification 01 57 13.
PPM-003
What are the vaccine and mask requirements for workers onsite?
Response: USDA Forest Service is following CDC guidance regarding masking and testing requirements when the Contractor enters an occupied Federal facility. Currently, guidance from the Safer Federal Workforce Task Force requires that different protocols for masking and testing for individuals not fully vaccinated be implemented based on the level of COVID-19 in the county where the USDA facility is located. If the community level for Deschutes County is high or medium, Attachment 6, Contractor Certification of Vaccination, and proof of vaccination or negative COVID-19 test from within three days, would need to be presented to enter the occupied facility. If community level is high, everyone, regardless of vaccination status, must wear a mask in the occupied facility.
PPM-004
Will the Paint and Haz Mat shed remain permanently located on the pad the contractors are required to pour?
Response: Yes the Paint and Haz Mat sheds will remain permanently located on the pad the contractors are required to pour. A temporary location for these two buildings may be required if the contractor desires to move them prior to concrete pad completion. Power service will be required to be provided to these two buildings in the temporary location.
Could the meters be relocated to the sheds themselves?
Response: Yes, this can be considered, however, it will require coordination with the Power Company
PPM-005
Will safety fencing be required around the construction site?
Response: No. However, the construction site will be required to comply with all OSHA standards.
PPM-006
Will the FS keep all the logs from trees needing to be cut?
Response: Yes. The Contractor will be required to limb and buck all logs above 7” D.B.H. and transport them to the USFS Whychus Pit on Forest Service Road 16. Approximate travel distance is 10 miles. All stumps, tops, limbs and other tree debris generated from tree clearing will be the responsibility of the Contractor to dispose of off Forest Service Land.

Will the Forest Service be providing temporary power and water for contractor use?
Response: No, Contractor will have to work with the electrical provider to establish a temporary power connection point and will have to work with the city of Sister Public Works Department to obtain a temporary meter to connect to one of the existing fire hydrants on the site.

Who is the Electrical provider?
Response: Central Electric Coop (CEC).
PPM-009
Is there a Geo Tech report for the site?
Response: Yes. A copy can be obtained by requesting one from Contract COR Chad Houchin at (541) 408-2870
PPM-010
Will electricity and concrete pads be required for the Storage Containers and where are they being relocated to?
Response: No electricity or concrete pads will be necessary for the Storage Containers. See attached Exhibit 01 for new Storage Container location. Forest Service will be responsible for removing contents of Storage Containers prior to Contractor relocating them.

Will the trenching of asphalt require repaving?
Response: No, all asphalt in this contract area will be replaced in a future phase of construction. Where asphalt is being required to be removed under this contract, compacted gravel will be put in it’s place.
Where concrete is required to be removed a sawcut line with clean edge will be required. Contractors required to use plates or gravel on trenching across bunkhouse driveways so residents have access to parking.

Where will the construction staging area be located?
Response: Once the contract is awarded, discussion can be had about staging and how much room the contractor will need.
PPM-013
Where will construction access to the site be located?
Response: Construction access will be from the north entrance to the property from Pine Street.
PPM-014
What is expectation of electrical to existing Residents onsite?
Response: Underground service is expected to the OUTSIDE of the houses as shown on the drawings.
Setting of new electrical panel and any internal building electrical work are not currently under the scope of this contract.
PPM-015
For the two new water services, can the contractor hot tap the water line or do they need to shut it down for connection?
Response: Hot tap will be ok. See revised Civil Drawing Sheet C3.1 attached to this amendment. The two new water services will be consolidated into one connection and connection is now to existing 10” C-900 water line that runs through property instead of 2” existing water line.

Is there Natural gas service on the site?
Response: No, there is no natural gas services to the site.
PPM-017
Occupancy of existing Residents onsite. Contractor to plan for occupancy of existing residences onsite.
Vehicle Access will be required to the residents with reasonable delays allowed due to required site and utility work.
PPM-018
Relocation of existing storage shed and car port (note #10 on Civil Drawing C1.2). Contractor will be responsible for disassembling covered car port and relocating car port to area identified on attached Exhibit 01. Re-assembly of car port will not be required. Other storage building identified by note #10 on Civil Drawing C1.2 will also be relocated by contractor to area identified on attached Exhibit 01.

K/A130 - Please verify if dry type sprinklers are required at canopies? No spec section provided.
Response: Fire Sprinkler are not required on this building, and therefore the canopies do not need to have fire sprinklers. The building is well under the maximum allowable area of 9000 sf which is the threshold where fire sprinklers would be required.

Will landscape/irrigation drawings be provided?
Response: Landscape is not in the scope of this project and will not be provided. Spec section have been removed from the spec book.

A101 - Will trash enclosure details be provided? Drawings note details on A101, none listed.
Response: The trash enclosure is not in the scope of this project and will not be provided.

C2/A210 - P-Lam wainscot @ Janitor. Please clarify is this is meant for FRP? If wainscot, please provide spec info.

C2.1 & A100 - Will updated civil drawings be released to reflect architectural site plan?
Response: No, to the design team’s knowledge the architectural site plan and civil plans match.

A100 - Are fuel and paint storage building included in this phase? If so, will drawings be released.
Response: the fuel and paint storage are not buildings. However, these are existing conex type storage containers that will be relocated during this phase.

2/E100 - Provide connection to future admin building. Admin building is not in scope, please clarify if meant for vault system connection. Civil drawing does not reflect this scope.
Response: The sheet keynote #2 should read “Provide conduit from storage building for future administration building connection to access door per manufacturer shop drawing requirements.
Coordinate with Civil drawings and Division 27.” Electrical drawing to be revised.
RFI-010
E100 - Primary Feed for utility shows the conduit going to UT Pole #102552. We can only figure what is shown on the drawings, please provide length or updated drawing.
Response: Refer to Civil drawing C3.1 for exact location of utility pole #102552.

1/E501 - Single Phase Service, the PUD booklet calls out a direct connect metering at the service entrance, are we to install this service as shown?
Response: Per CEC, a CT cabinet is required at services 400A (continuous) or larger.
RFI-012
E501 - Feeder from the X-frmr to the terminal cabinet (400A) is calling out conductors by PUD and installed by contractor, this is not standard procedure, is this note correct? Please clarify.
Response: Per discussion with CEC, secondary conductors furnished and installed by PUD.

E702 - RV Meter Pedestal - What manufacturer and model is called for? Please clarify. Very few designs available.
Response: RV Meter Pedestal manufacturer and model noted on site plan E100, sheet keynote #6.

4/T100 - Note references sheet A103 for more information, no A103 sheet provided. Will this sheet be provided?
Response: The reference has been removed. There is not a sheet A103 in the set.
RFI-015
Page A100 states that the landscaping scope does not occur until Phase 3 construction. However, there is a spec for landscaping and irrigation in the spec book. Is there no landscaping scope during this phase?
Is gravel the finish material for the active construction area during this phase?
Response: No landscaping or irrigation is required in this contract and associated specs have been removed. Gravel is the finish material for this contract

Page A100 calls out a gate and perimeter fence. However, there is no spec for these exterior improvements. Is the Gate and Fencing part of a later phase in this project?
Response: No gates or fences are part of this contract. They are part of a later phase of construction.

Do you want us to supply the Air compressor (AC-10) that appears on P002 under Misc Equipment Schedule, or will this be OFCI?
Response: Air Compressor to be CFCI
RFI-018
How long to you want to logs to be that we are moving off site to the forest pit?

Do the 1.06F Air Contaminant Test Plan and the 1.06G Air Contaminant Test Report refer to the completed/post construction HVAC system and the air quality for the Forest Service Staff occupying the structure. Or will testing be required during the construction period as well?
Response: Test Reports and Testing refer to completed/post construction HVAC system and air quality.
RFI-020
There is no reference to an Environmental Protection Plan (EPP). Will an EPP be required for this project?
Response: A NPDES permit is not required however Contractor will be required to develop and submit an Erosion Control and Sediment that meets EPA requirements. See specification 01 57 13.
RFI-021
We noticed that a TP-1 is called out in the mechanical room, but it’s not in the fixture/equip schedule. We didn’t see it in the mech/elec schedules either. Our guess is that it’s an electronic trap primer servicing the floor drains. Can you please clarify?

RFI-022
Per finish legend. Manufacturer to be Beuner Block which is located in Salt Lake City Utah. Please confirm local supplier Willamette Graystone or Mutual Material to be approved supplier.
Response: No substitutions are allowed prior to bidding. Substitutions of materials that meet the salient characteristics of the basis of design in the contract will be considered and likely approved after contract award.
Color to be Natural Gray. Please provide texture and confirm 4” thickness, not 8”,10” & 12” as noted Response: thickness to be 4”. Texture to be Split Face

The solicitation mentions the requirement for the contractor to “perform construction practices and reporting that help obtain a Green Building certification.” Will the contractor be required to provide a certification (to include a Green Globes Assessor), or simply to use biobased and recovered materials and report those to the Forest Service?
Response: The Owner and Designer have already engaged and paid the Green Globes fee and an Assessor has already been assigned to the project. Contractor will be responsible for following the Contractor requirements identified in the Specifications.

There is fencing shown on the civil plans but no spec section or drawing details. Can the USFS provide these?
Response: No gates or fences are part of this contract. They are part of a later phase of construction.
RFI-025
The finish schedule on the drawings details out wall paint but makes no mention of the paint for the ceilings. Please clarify.
Response: A120 Finish Plan - the room finish schedule is for walls only. However, our Finish Legend calls ceiling paint out as PT5. A130 Reflected Ceiling Plan – Calls out PT5 on the legend

Note: Executive Order (EO) 13706, Establishing Paid Sick Leave for Federal Contractors applies to all contracts subject to the Davis-Bacon Act for which the contract is awarded (and any solicitation was issued) on or after January 1, 2017. If this contract is covered by the EO, the contractor must provide employees with 1 hour of paid sick leave for every 30 hours they work, up to 56 hours of paid sick leave each year.

The body of each wage determination lists the classification and wage rates that have been found to be prevailing for the cited type(s) of construction in the area covered by the wage determination. The classifications are listed in alphabetical order of ""identifiers"" that indicate whether the particular rate is a union rate (current union negotiated rate for local), a survey rate (weighted average rate) or a union average rate (weighted union average rate).
Union Rate Identifiers
A four letter classification abbreviation identifier enclosed in dotted lines beginning with characters other than ""SU"" or ""UAVG"" denotes that the union classification and rate were prevailing for that classification in the survey. Example:
PLUM0198-005 07/01/2014. PLUM is an abbreviation identifier of the union which prevailed in the survey for this classification, which in this example would be Plumbers. 0198 indicates the local union number or district council number

The next number, 005 in the example, is an internal number used in processing the wage determination. 07/01/2014 is the effective date of the most current negotiated rate, which in this example is July 1, 2014.
Union prevailing wage rates are updated to reflect all rate changes in the collective bargaining agreement (CBA) governing this classification and rate.
Survey Rate Identifiers
Classifications listed under the ""SU"" identifier indicate that no one rate prevailed for this classification in the survey and the published rate is derived by computing a weighted average rate based on all the rates reported in the survey for that classification. As this weighted average rate includes all rates reported in the survey, it may include both union and non-union rates. Example: SULA2012-007 5/13/2014. SU indicates the rates are survey rates based on a weighted average calculation of rates and are not majority rates. LA indicates the State of Louisiana. 2012 is the year of survey on which these classifications and rates are based. The next number, 007 in the example, is an internal number used in producing the wage determination. 5/13/2014 indicates the survey completion date for the classifications and rates under that identifier.
Survey wage rates are not updated and remain in effect until a new survey is conducted.
Union Average Rate Identifiers
Classification(s) listed under the UAVG identifier indicate that no single majority rate prevailed for those classifications; however, 100% of the data reported for the classifications was union data. EXAMPLE: UAVG-OH-0010 08/29/2014. UAVG indicates that the rate is a weighted union average rate. OH indicates the state. The next number, 0010 in the example, is an internal number used in producing the wage determination. 08/29/2014 indicates the survey completion date for the classifications and rates under that identifier.
A UAVG rate will be updated once a year, usually in January of each year, to reflect a weighted average of the current negotiated/CBA rate of the union locals from which the rate is based.
WAGE DETERMINATION APPEALS PROCESS
1.) Has there been an initial decision in the matter? This can be:
* an existing published wage determination
* a survey underlying a wage determination
* a Wage and Hour Division letter setting forth a position on a wage determination matter
* a conformance (additional classification and rate) ruling
On survey related matters, initial contact, including requests for summaries of surveys, should be with the Wage and Hour National Office because National Office has responsibility for
