About this file

This is a Human Trafficking Attestation form required by the City of Tallahassee for nongovernmental entities executing, renewing, or extending contracts with the municipality. The attestation, mandated under Section 787.06(13) of Florida Statutes, pertains to the Forestmeadows Tennis Courts 17 & 18 Conversion Project located at 4750 Meridian Road, Tallahassee, Florida. The form requires an officer or authorized representative of the contracting entity to affirm under penalty of perjury that the entity does not use coercion for labor or services as defined by Florida law. The attestation must be completed and signed by an authorized representative and submitted as part of the contract execution, renewal, or extension process.

The attestation form establishes compliance requirements and ongoing obligations for contractors. The signatory certifies that they are over eighteen years of age, authorized to represent the entity, and knowledgeable of the facts stated in the attestation. The contractor must commit to immediately notifying the City of Tallahassee if the entity engages in coercion for labor or services at any point during the contract term, with the understanding that such action would preclude future contract execution, renewal, or extension. Failure to provide truthful attestation subjects the responsible party to criminal penalties under Florida law. The form is version 4/10/25 and serves as a mandatory compliance document for all nongovernmental entities contracting with the City of Tallahassee.
