About this file
This is an Equipment Vendor Post Installation Quality Assurance Inspection Checklist and Punch List Form for Dallas Independent School District (DISD) in Texas, related to an Uninterruptible Power Supply (UPS) procurement. The form serves as a quality assurance tool for evaluating installed equipment across various network cable categories and fiber optic specifications, including Cat 5e, Cat 6, Cat 6A, armored and non-armored cables, multimode fiber (MMF OM2 and OM4), and single-mode fiber (SMF OS2 and OS1). The UPS procurement is intended for district-wide use on an as-needed basis and will be administered in accordance with Title 2 of the Code of Federal Regulations Part 200 requirements. The resulting contract will have an initial three-year term with two successive one-year renewal options available, not to exceed five years total, contingent upon the district's long-range needs and mutual agreement with the awarded vendor. Either party must provide written notice at least ninety days prior to contract expiration if renewal is not intended.
The procurement is available to all vendors, including sole source providers, and may be funded through local, state, federal, and grant-funded programs. The checklist document specifically references a 50 micron/1Gbps specification line item, indicating technical performance standards for the installed infrastructure. No specific pricing terms, set-asides, incumbent information, or funding amounts are detailed in the provided documentation.
