About this file

This is a Public Works Contractor (PWC100) registration and information form issued by the California Department of Industrial Relations (DIR) in compliance with Senate Bill 854, which established a mandatory public works contractor registration program effective June 20, 2014. The form is used to collect registration information for prime contractors and all subcontractors intending to bid on or perform work on public works projects, including the referenced Hazardous Waste Removal Services project managed by Wm. S. Hart Union High School District. Contractors must provide company information, contact details, CSLB license numbers, DIR registration numbers, and certification of their trade classifications, which include options such as Asbestos, Carpenters, Electricians, Laborers, Operating Engineers, and other skilled trades relevant to the specific project scope.

All public works contractors are required to register with the DIR program and annually renew their registration at fees of $400 for one fiscal year, $800 for two fiscal years, or $1,200 for three fiscal years. Beyond registration, contractors must fulfill four key responsibilities: pay prevailing wages, follow apprenticeship requirements, maintain certified payroll records, and submit those records to the DIR. For the Hazardous Waste Removal Services package (HAZ2604A), the contracting entity must possess an active class-type license (A, B, C-12, C-21, C-36, C-57, or C-61/D40) along with a HAZ-Hazardous Substance Removal Certification. The form requires submission of a separate PWC100 for each prime contractor and subcontractor, with designated personnel responsible for uploading certified payroll records and project documentation to the Division of State Architect (DSA) Box.
