About this file
This is a PWC100 and DSA Box Upload Information Form issued by the California Department of Industrial Relations (DIR) in compliance with Senate Bill 854, which establishes mandatory registration and compliance requirements for all public works contractors and subcontractors. The form serves as a registration and information submission document for contractors intending to bid on or perform work on public works projects, including the William S. Hart Union High School District's Hazardous Waste Removal Services contract (Package HAZ2604A). All contractors and subcontractors must submit a separate PWC100 form and provide project-specific information including company name, contact details, CSLB license number, DIR registration number, and applicable worker classifications. The form requires identification of trade classifications relevant to the project, with options including asbestos removal, carpentry, electrical work, and various other skilled trades. Sealed electronic bids for the hazardous waste removal services contract are due by 11:00 a.m. on April 29, 2026, with contract award anticipated on May 20, 2026, and services commencing on July 1, 2026, for an initial three-year term with two successive one-year renewal options available.
Registration fees for public works contractors are $400 for one fiscal year, $800 for two fiscal years, or $1,200 for three fiscal years, and all registered contractors must comply with four key responsibilities: maintaining current registration, paying prevailing wages, meeting apprenticeship requirements, and submitting certified payroll records to DIR. The form requires submission of contact information for the person responsible for uploading certified payroll records (CPRs) and, separately, the person responsible for uploading documentation to the Division of the State Architect (DSA) Box. Contractors bidding on this public works project must possess one of the specified active California contractor licenses (Class A, B, C-12, C-21, C-36, C-57, or C-61/D40), a Hazardous Substance Removal (HAZ) Certification, and a California Highway Patrol Hazardous Materials License with current 40-hour HAZPOWER Certificates and annual refresher training. Bid security of 10 percent of the maximum bid and Performance and Payment Bonds each equal to 100 percent of the contract price are required, along with comprehensive insurance coverage including $2,000,000 Commercial General Liability and $2,000,000 Pollution Liability minimums.
