The PWC100 form requires contractors to provide detailed company information, CSLB license numbers, DIR registration numbers, and email contacts for both the person submitting certified payroll records and the person responsible for uploading documentation to the Division of the State Architect (DSA) box. All contractors and subcontractors must register with DIR and renew annually, with registration fees of $400 for one fiscal year, $800 for two years, or $1,200 for three years. Contractors must comply with prevailing wage requirements, apprenticeship obligations, and certified payroll record submission requirements as mandated under California Labor Code Section 1725.5. The form includes checkboxes for multiple worker classifications applicable to the public works project, including Carpenters, Electricians, Laborers, Operating Engineers, and other skilled trades. Detailed information is available through the DIR website at https://www.dir.ca.gov/?Public-Works/Contractors.html for contractors seeking guidance on registration procedures, prevailing wage determination, and compliance monitoring and enforcement protocols.
