About this file

This is a Pre-Employment or Pre-Service Affidavit form required by the Texas Education Code (TEC) 22A.055 for individuals applying for employment or service provider positions with educational entities, including school districts, charter schools, and regional education service centers. The form requires disclosure of prior employment history with educational institutions, consent for release of employment records, and comprehensive background information regarding any termination, non-renewal, discharge, or resignation from educational positions. Applicants must also disclose any investigation, charges, adjudication, or conviction involving unlawful conduct with students or minors, including physical mistreatment, threats of violence, romantic or sexual contact, inappropriate communications, or boundary violations as defined by board rule. Additionally, applicants must report any denial, suspension, revocation, or sanction of licenses, certificates, or permits by licensing authorities, as well as any current inquiries, disciplinary actions, or investigations by schools, licensing agencies, law enforcement, or courts related to alleged misconduct.

Failure to disclose required information constitutes a Class B misdemeanor offense and serves as grounds for termination of employment or service. The form requires detailed disclosure of all relevant facts known to the applicant for any affirmative responses to the background questions. While this affidavit form is not directly related to the Nutritious Snack Foods for Food and Child Nutrition Services contract opportunity for Dallas ISD, it may be applicable to individuals hired to fulfill service-related duties under any resulting contract with the district, as Dallas ISD is classified as an educational entity subject to these disclosure requirements.
