About this file

This is a Bidder Response Template (Exhibit 1) for Request for Proposals EV00000945 issued by the Oklahoma Department of Mental Health and Substance Abuse Services (ODMHSAS) for Community Based Prevention Services. The RFP solicits proposals from qualified organizations to implement evidence-based substance misuse prevention and mental health promotion services across 39 Oklahoma counties and county-specific focus areas (including separate cannabis and opioid initiatives in Tulsa and Oklahoma counties). Bidders must provide comprehensive responses addressing community identification and demographic profiling, documentation of service needs and gaps, relationships with Certified Community Behavioral Health Clinics (CCBHCs) and Federally Qualified Health Centers (FQHCs), community partnership strategies, project implementation goals and measurable objectives, required activities including needs assessments and logic models, first-year realistic timelines, organizational capacity and staffing qualifications, and evaluation and data collection methodologies. Mandatory submission requirements include completed OMES forms (CP Form 076 and CP Form 004), signed amendments, certificates of insurance meeting specified standards, disclosure of conflicts of interest, and letters of commitment from partner organizations. Bidders may submit responses for single or multiple counties, with incomplete submissions for specific counties subject to disqualification.

The template requires bidders to complete all sections fully, with blank responses resulting in disqualification from further evaluation. Bidders must identify whether any portion of their bid should be held confidential and disclose any requested exceptions to contract terms using tracked changes in Word format. Key personnel positions must be identified, including a Project Director and Data Analyst, with detailed descriptions of roles, levels of effort, and qualifications for serving target populations. The submission format utilizes yellow response boxes with provisions for clearly labeled supplemental responses if needed. All bidders must maintain active OMES Supplier Registration and provide tax identification numbers, Secretary of State filing numbers, and designated contact personnel for contract administration and post-award management. The template indicates that Sections 9 (Value-Added Products), 10 (Financial Information), and 12 (3rd Party Supplier Information) are intentionally omitted, with pricing information to be submitted separately via the Cost Template Checklist (Exhibit 2).
