About this file

The document is a Certified Drug-Free Workplace Report Coversheet issued by the State of West Virginia Purchasing Division for a contract with Welch Community Hospital. The specific contract (ARFQ WEH25000007) is for an HVAC unit in the hospital's laboratory department. The form requires contractors to submit a detailed report at least annually or upon project completion, documenting drug testing and workplace education efforts in compliance with West Virginia Code § 21-1D-7b.

For contracts over $25,000, the report must be submitted to the West Virginia Purchasing Division at 2019 Washington Street East in Charleston. The required report content includes information about employee education and training, the certified drug testing laboratory, average number of employees, and comprehensive drug test results across four categories: pre-employment/new hires, reasonable suspicion, post-accident, and random testing. The coversheet provides a structured template for vendors to document their drug-free workplace compliance and testing protocols.
