About this file

ENG Form 6213 (August 2024) is a U.S. Army Corps of Engineers template for documenting critical lift plans for load handling equipment and crane operations. The form is designed to identify, evaluate, and address safety requirements before executing any lifting activity, and it must be completed by a competent person with all required inspections, tests, and preparation requirements verified prior to lift commencement.

The form requires comprehensive documentation across eight main sections: total load weight (including auxiliary blocks, main blocks, lifting beams, slings, jibs, and hoist rope); operator qualifications (certification, type/class/capacity certification, and written employer designation); crane specifications (type, capacity, radius, boom length, boom angle, and jib information); a detailed pre-lift checklist covering inspection, setup, hazard assessment, communication, and meeting requirements; hoist rope specifications (number of parts, diameter, and capacity for main and auxiliary systems); rigging details (hitch types, sling specifications, shackle information); and for mobile cranes, crane placement considerations including bearing pressure, ground conditions, electrical hazards, and obstructions. The form includes designated signature blocks for the crane operator, rigger(s), signal person, lift director, and other personnel, along with attachable pages for load calculations, bearing pressure and ground condition analysis, load charts, operator/rigger/signal person qualifications, and site plans with sequencing. The form references compliance with EM 385-1-1 and identifies CESO as the proponent agency.
