About this file

This is a Quality Final Inspection Report for the MV SAM Survey Boat conducted by World Testing, Inc. on October 31, 2025, with final report issued November 4, 2025. The inspection was performed for the US Corps of Engineers at 1188 Cinder Road, Old Hickory, Tennessee, with contact Jeremy Doughtry at (904) 382-5947.

The inspection encompassed visual and ultrasonic thickness evaluation of the boat hull and associated components. Ten significant discrepancies were identified and documented with photographic evidence: (1) forward hull bottom doubler plate crack, (2) starboard hull side weld seam porosity, (3) port hull side weld seam porosity, (4) starboard bow crack and dent, (5) starboard stern dent, (6) port stern dent, (7) port hull side gashes, (8) trailer framing crack, (9) starboard internal weld seam lack of fusion, and (10) port bow weld seam porosity. Ultrasonic thickness measurements ranging from 0.176 to 0.188 inches were recorded across four measurement locations (port side, port lower, starboard lower, and starboard side) with eight readings taken from bow to stern at each location. All discrepancies were marked on the hull in yellow marker and on the trailer in black sharpie. The inspection conclusion was "Refer to Narrative Report," indicating the vessel does not meet acceptance criteria without remediation of the identified defects. Inspector Chris Ennis, CWI, conducted the inspection under World Testing, Inc. Catalog No. 91296.
