AMENDMENT 0002
THE FOLLOWING CHANGES ARE MADE TO SECTION L:
Question 1 - Instructions only call for PPQs delivered directly to the CO - section M additionally calls out adding demonstrating corrective actions. There is no PPQ form attached to the RFP.
Additionally there is a 25 page limit for this section with no instructions for what the 25 pages should contain. Assuming project descriptions, maximum of 5, 5 pages each for 25 pages total?
No PD form attached to the RFP. Please advise specific instructions form(s), and what should be contained in the 25 pages.
ADDED BY AMENDMENT 0002 - Offerors may submit all Government and/or commercial contracts for the prime Offeror and each major subcontractor in performance or awarded during the past ten (10) years, from the issue date of this RFP, which are relevant to the efforts required by this solicitation. Data concerning the Offeror shall be provided first, followed by each proposed major subcontractor, in alphabetical order. The Offeror may also submit the written consent of its major subcontractors to allow the disclosure of its subcontractor’s past performance information to the Offeror. No past performance information shall be contained in any other volume of this proposal. The Offeror may also submit the written consent of its major subcontractors to allow the disclosure of its subcontractor’s past performance information to the Offeror. No past performance information shall be contained in any other volume of this proposal.
Include performance recognition documents received within the last 3 years such as awards, award fee determinations, customer letters of commendation, and any other forms of performance recognition.
Large Businesses Only - Describe the extent to which you attained applicable small business subcontracting goals for contracts that required you to submit a Subcontracting Plan. You may provide copies of not more than six Individual Subcontracting Reports (ISRs) or Summary Subcontracting Report (SSR)) to validate your past performance. Provide an explanation of your efforts where you failed to meet goals. You may also include documentation to support Joint/Venture, mentor/protégé or similar teaming and partnering agreements with small businesses.
Submit documentation to support the assessment of the contractor's past performance for small business and the extent to which the offeror’s past performance demonstrates compliance with meeting/exceeding subcontracting plan goals. Pursuant to DFARS 215.305(a) (2), the assessment will also consider the extent to which the Offeror’s past performance demonstrates compliance with FAR 52.219-8; Utilization of Small Business Concerns and FAR 52.219-9, Small Business Subcontracting Plan.
While the Government may elect to consider data from other sources, the burden of providing detailed, current, accurate and complete past performance information rests with the Offeror.
PPQ has been added to FBO as an attachment.
Question 2 - The Introduction to Section L on Page 99 reads: "The term 'offeror' is defined as a single offeror, joint venture, or team of offerors and major sub-contractors."
Section (ii) VOLUME II – FACTOR II – PAST PERFORMANCE on Page 103 states: "The offeror may submit up to five (5) Past Performance Questionnaire (PPQ) Forms."
Q: Will projects performed by a mix of a single Prime/JV partners and named major subcontractors (i.e., the proposed team) all be rated the same in relevancy and/or confidence? If not, please provide details on how varying projects will be scored.
ADDED BY AMENDMENT 0002 - A relevancy determination of the offeror’s present and past performance, including joint venture partners and/or major subcontractors will be made. In determining relevancy for individual citations, consideration will be given to the effort, or portion of the effort being proposed by the offeror, joint venture partner, or subcontractor whose citation is being reviewed and evaluated. The Government is not bound by the offeror’s opinion of relevancy. The relevancy table and data obtained from other sources will be considered in establishing the relevance of present and past efforts. Relevancy contemplates contracts of a similar magnitude, nature and complexity to include dollar value and contract type. All performance data obtained will be reviewed and evaluated to determine how closely the work performed under those relates to the proposed efforts. Past and present performance data identified by offerors in their proposals will be confirmed, as deemed necessary. The Government may consider as relevant efforts performed for other agencies of the federal, state or local governments and commercial customers.
Question 3 - We request that cover sheets, tables of contents, cover letters, acronym lists, and tabs/dividers NOT be included in the page counts for Volumes I-III. Please confirm that these items will be excluded from the count.
ADDED BY AMENDMENT 0002 - The title page, table of contents, list of tables and glossary of abbreviations & acronyms do not count against page count limitations. Proposal contents that exceed the stated page limitations will be removed from the proposal by the Contracting Officer, prior to turning the proposal over to the Government evaluation teams, and will not be considered in the evaluation.
Question 4 - Page 103 states "Questionnaires shall be submitted directly from client to Government Representative" however, the PPQ form states "The Offeror will include it in proposal"
ADDED BY AMENDMENT 0002 - Completed Past Performance Questionnaires should be submitted with your proposal. If the offeror is unable to obtain a completed PPQ from a client for a project(s) before proposal closing date, the offeror should complete and submit with the proposal the first page of the PPQ (Attachment), which will provide contract and client information for the respective project(s). Offerors should follow-up with clients/references to ensure timely submittal of questionnaires. If the client requests, questionnaires may be submitted directly to the Government’s point of contact, Sally A. Duncan via email at sally.a.duncan@usace.army.mil prior to proposal closing date.
