SUMMARY OF CHANGES
SECTION A - SOLICITATION/CONTRACT FORM
The required response date/time has changed from 18-Jun-2019 02:00 PM to 19-Jun-2019
02:00 PM.
SECTION L - INSTRUCTIONS, CONDITIONS AND NOTICES TO BIDDERS
The following have been modified:
AMENDMENT 0010
Section L – Instructions, Conditions and Notice to Bidders
PROPOSAL SUBMISSION:
1. INTRODUCTION
The term “offeror” is defined as a single offeror, joint venture, or team of offerors and major sub-contractors. Proposals submitted under a formal partnering agreement or joint-venture must be accompanied by a copy of the legal document establishing the agreement or joint venture.
An offeror may receive multiple awards. There is no limit on the number of awards an offeror may receive. To propose for more than one region, the offeror must submit separate and complete proposals in accordance with instructions in Section L of the solicitation for each region. Offerors must identify the specific regions and indicate whether the proposal is for Suite 1- Unrestricted, or Suite 2 – Restricted. The offeror’s proposal must also convey how multiple missions in separate regions will be executed simultaneously if the offeror is awarded multiple regions. The schedule of prices must represent the region being proposed. Each proposal submitted will be evaluated separately.
ADDED BY AMENDMENT 0006
Offerors who want to be considered for multiple contract awards must propose discrete Key Personnel Teams for each region. Each proposed Key Personnel Team will be evaluated separately. If the offeror is selected for award, the team proposed will be unavailable to be considered for additional awards because they will be assigned to that contract. Offerors must rank all proposed Key Personnel Teams in order of precedence (specify Suite 1 or Suite 2 for every Region for which a proposal is submitted) so the Government understands how to consider their proposals.
The contractor must rank precedence of region in the offeror’s preferred order of proposal evaluation by the Government. Below is a sample template that may be used to rank regions/key personnel teams.
Suite 1 or 2 Region Ranking Team Ranking
Suite 1 Region 2 Team 1 Suite 1 Region 5 Team 2 Suite 1 Region 8 Team 3 Suite 1 Region 7 Team 4 Suite 2 Region 12 Team 5
Please refer to Section M, TABLE 3 – Order of Contract Awards by Regions of the solicitation which describes the order in which the Government will evaluate and award the regions. Using the template above as an example, when evaluating Suite 1, Region2, the Government would evaluate the Offeror’s proposal with Team 1 as the highest ranking preferred team to be assigned to that region. If the Offeror were to be awarded Suite 1, Region 2, Team 1 would not be available to be used on Suite 1, Region 5, or any other region or suite.
In the above example, if the Offeror was not awarded Region 2, then Team 1 would be the highest ranked team and would be evaluated for award for Region 5.
All awarded contracts must have unique Key Personnel Teams. For example: If Joe Smith is listed as Operations Manager for offeror A, Team 1, then Joe Smith cannot be listed as Operations Manager on any other Teams submitted by offeror A.
END OF AMENDMENT 0006
Offerors must submit fixed price and escalation rates for every Region (specify Suite 1 or Suite 2 for every Region for which a proposal is submitted) for which they wish to be considered. These prices will be contractually binding and cover each contract line item included in Section B of the solicitation.
It is the offeror's responsibility to ensure the completeness and accuracy of his or her proposal. Corrections will not be made by the Government.
The written proposal will be the basis on which the Government will determine the capability of the Offeror to perform this work. The proposal must be clear, specific, detailed, and demonstrate that the Offeror has a thorough understanding of the solicitation requirements and capability to execute. The proposal must address all requirements specified in Section L.
In addressing the requirements of Section L, the proposal must not merely offer to comply with the Government’s requirements by simply restating them, but must specifically and thoroughly describe the means of accomplishment. Offerors must include sufficient details to permit a complete and accurate evaluation of each proposal. “Parroting” of the technical requirements with a statement of intent to perform does not reflect an understanding of the requirement or capability to perform.
Proprietary information must be clearly marked. Files must not contain classified information.
The use of hyperlinks in proposals is prohibited and will be a basis for being declared non-responsive. The Offeror must adhere to the proposal pages limitation.
2. PROPOSAL SUBMISSION REQUIREMENTS

b. Content Requirements. All information must be confined to the appropriate volume. The offeror must confine submissions to essential matters, sufficient to define the proposal, and must provide adequate basis for evaluation. Each volume of the proposal must consist of a table of contents, a summary section, and the narrative discussion. The summary section must contain a brief abstract of the volume. Proprietary information must be clearly marked. The Government will not assume, nor consider, anything that is not specifically addressed in the proposal. No proposal price data must appear in any volume other than Volume IV, Price. The following must be included in the narrative discussion:
(i) VOLUME I – FACTOR I – MANAGEMENT/TECHNICAL APPROACH. Volume 1 must consist of all information required to support the management/technical approach and must not exceed seventy five (75) pages. ADDED BY AMENDMENT 0008
Sub-factor A – Key Personnel and Staffing Approach THE KEY PERSONNEL TEAM
RESUMES AND COMMITMENT LETTERS MUST BE IN VOLUME I AND WILL ONLY BE
EVALUATED ONCE. THE RESUMES AND COMMITMENT LETTERS DO NOT COUNT
TOWARDS VOLUME I PAGE COUNT.
THOSE OFFEROS WHO WANT TO BE CONSIDERED FOR MULTIPLE CONTRACT
AWARDS MAY PROPOSE THE KEY PERSONNEL TEAMS IN MULTIPLE PROPOSALS
USING THE INSTRUCTIONS BELOW.
a. Key Personnel – Provide an organization chart to include key personnel in the disciplines listed below:
1) Operations Manager
2) Quality Control Manager
3) Environmental Health and Safety Manager
4) Operations Planner
Proposals must provide a description of previous experience on projects of similar complexity for the key personnel to be assigned to this contract. The proposal must also identify why the key personnel proposed are best qualified by addressing their knowledge, skills, security clearances, education, and experience. Include resumes and commitment letters for Key Personnel listed above. Resumes must be an attachment to Volume 1 but will not count against the page count. Resumes shall address each individual’s current and past job experience. Letters of Commitment must be an attachment to Volume 1, but will not count against the page count.
Letters of Commitment must be signed by the individual of the resume submitted. The commitment letter must demonstrate that the key personnel understand the commitment, will accept the employment, and the understanding that this commitment is for the duration of the contract.
The ranking of Key Personnel Teams must be contained in Volume I. The contractor must rank precedence of region in the offeror’s preferred order of proposal evaluation by the Government.
Below is a sample template that may be used to rank regions/key personnel teams.
Suite 1 or 2 Region Ranking Team Ranking Suite 1 Region 2 Team 1 Suite 1 Region 5 Team 2 Suite 1 Region 8 Team 3 Suite 1 Region 7 Team 4 Suite 2 Region 12 Team 5
The offeror must utilize the key personnel proposed if awarded the contract for the specific region. Further, these personnel will no longer be considered as “available” key personnel in any other offers by this prime contractor, because under the contract awarded, these personnel have been identified to the region following the order of awards outlined in Section M, Table 3.
Substitution of any key personnel will require prior approval of the Contracting Officer after contract award.
ORGANIZATIONAL CHART - The organization chart must 1) clearly identify on-site and management personnel including lines of authority and 2) list subcontractor’s primary personnel including lines of communication. The organizational chart must clearly illustrate the offeror’s understanding of each aspect of the process. (Resumes and commitment letters are not required of subcontractors)
END OF AMENDMENT 0008 ADDITIONS
b. Staffing Approach - The offeror must identify core resources and/or capabilities which enable it to efficiently meet the immediate Government requirements after Notice to Proceed (NTP) of a Task Order. The offeror must discuss accommodating fluctuating workloads, simultaneous debris removal task orders, minimizing turnover, and retaining qualified personnel while demonstrating the capability to meet, sustain, and manage geographic areas relating to sectoring, sector management and resources as needed to successfully perform the Task Order.
The term sectoring indicates a contractor’s ability to create sectors for the most efficient debris management. The offeror must describe how major subcontractors fit into its overall staffing approach. The offeror must describe its plan to ensure that the workforce is qualified and capable of fulfilling the contractual requirements.
Offerors are required to convey how they plan to prepare and execute concurrent missions in different areas of a specific region in accordance with the Performance Work Statement (PWS).
Additionally, if the offeror is proposing on multiple regions, offerors must also explain how they will handle concurrent missions in different regions. The Government will evaluate the offeror’s ability to handle concurrent missions in multiple regions.
Sub-factor B – Readiness for Debris Removal Services
a. Deployment/Mobilization - The offeror must demonstrate its intended approach to deploy and mobilize personnel and resources at both the start of work and at task order completion. The transition plan must include, at a minimum, the offeror’s plan for the rapid deployment, formation of a contingent workforce, and appropriate in-processing activities (e.g., permits, immunizations, training, etc.). Offerors must submit a narrative description of the offeror’s means and methods to provide real-time mission execution monitoring that compliments the offeror’s system for meeting the requirements in the PWS.
CORRECTED BY AMENDMENT 0010
b. Management/Operations Plan - The Management/Operations Plan will not count towards the overall Volume I page count. The Management/Operations Plan must be added as an Appendix to Volume I. The offeror must describe its plan to manage on- and off-site support personnel without Government interference throughout the period of performance. The offeror must include a description of quality control processes and procedures planned (to include any certifications and quality management systems used), as well as the quantitative metrics intended, to execute and monitor compliance with contractual requirements. The offeror must provide a strategy to address retention of support personnel throughout the period of performance. The strategy must include a description of the offeror’s mitigation plan to address assigning personnel by the Notice to Proceed date, the unavailability of equipment and disposal sites, surge requirements, and poor performance. The offeror is encouraged to provide descriptions and illustrations which show the contractors and subcontractors qualifications and methodology to be followed during a response to an emergency debris removal mission and demonstrate the offeror’s approach to ensure the performance objectives outlined in the PWS are met. The offeror must demonstrate management of geographic areas relating to sectoring, sector managers, numbers of crews in each sector and haul destinations from each sector. The intent is for the offeror to describe how they will manage their support personnel that will be designated to the work effort. It is up to the offeror to determine if a lodging/feeding plan is applicable to the requirement. On site personnel are personnel at the debris site supporting the on-going mission. Off-site personnel are personnel who would remain at the contractors home office, but would be supporting the mission.
c. Safety – The offeror must provide a Corporate Safety plan. The Corporate Safety plan must include, safety positions, including safety competent persons and duties, must be outlined noting compliance with the duty requirements of the PWS. The contractor must provide documentation that demonstrates their safety and health program experience for the past five (5) years.

(ii) VOLUME II – FACTOR II – PAST PERFORMANCE
ADDED BY AMENDMENT 0008
This volume must contain past performance information regarding similar contracts. This volume must not exceed 25 pages plus five pages for each major subcontractor, excluding Past Performance Questionnaire Forms. Major subcontractors are defined as members of an offeror’s overall team who are expected to perform ten (10) percent or more of the proposed effort.
Offerors must submit all Government and/or commercial contracts for the prime offeror and each major subcontractor in performance or awarded during the past ten (10) years, from the issue date of this RFP, which are relevant to the efforts required by this solicitation. Relevant efforts are defined as services/efforts that are the same as or similar to the effort (as compared to NAICS code 562119) required by the RFP. Data concerning the offeror must be provided first, followed by each proposed major subcontractor, in alphabetical order. The Offeror must also submit the written consent of its major subcontractors to allow the disclosure of its subcontractor’s past performance information to the Government.

The Government may consider a wide array of information from a variety of sources, but is not compelled to rely on all of the information available.
In addition, letters of commitment must be included for all major subcontractors for their past performance to be considered. The letters of commitment from major subcontractors are not counted towards the page count for Volume II. This volume must be organized into the following sections:

The offerors must list each time the delivery schedule was revised and provide an explanation of why the revision was necessary. All Requests for Deviation and Requests for Waiver must be addressed with respect to causes and corrective actions. The offerors must also provide copy of any Cure Notices or Show Cause Letters received on each contract listed and a description of any corrective action implemented by the offeror or proposed subcontractor. The offerors must indicate if any of the contracts listed were terminated and the reasons for the termination.

The offeror must complete Part I of the Past Performance Questionnaire and the client or technical representative responsible for the past contract will complete Part II and provide the completed form to the offeror. The POCs must be instructed to electronically complete Part II of the questionnaire and provide back to the offeror. Completed Past Performance Questionnaires must be submitted with your proposal. If the offeror is unable to obtain a completed PPQ from a client for a project(s) before proposal closing date, the offeror should complete and submit with the proposal the first page of the PPQ (Attachment), which will provide contract and client information for the respective project(s). Offerors should follow-up with clients/references to ensure timely submittal of questionnaires.
CORRECTED BY AMENDMENT 0008
(5) Submissions. Offerors are discouraged from providing points of contact with another contractors’ facility, i.e., in case an offeror (or one of his/her team members) is in a subcontract with another contractor who has submitted a proposal on the same requirement.
Offerors must provide and submit the prime contract number and all Governmental agency points of contact (POC) in lieu of subcontract numbers or prime contract POCs in situations as described above.
(5) If applicable, include information on organization structure change history that occurred in the past ten years, or a joint venture resulting in a different company name. Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant past efforts or between conclusion of recent past efforts and this source selection.
While the Government may elect to consider data from other sources, the burden of providing detailed, current, accurate and complete past performance information rests with the Offeror.
