NOTES

1. Any bid which is materially unbalanced as to the price for the individual bid items may be rejected. An unbalanced bid is one which is based on prices significantly less than cost for some work and prices significantly overstated for other work.

31 41 16 Sheet Piling

Added definition of “SPT Refusal” to paragraph 3.2.2.2.e.

(Questions that may be of general interest to all Offerors and/or the Government and that are not readily answered by the proceeding changes will appear below. These questions may have been paraphrased or altered to represent several questions regarding the same subject and/or simply revised to clarify and simplify the question(s).

Questions and answers are issued to the Offerors “for information only ”and do not revise the solicitation, plans or specifications.).

Question 1: In Amendment 2, a requirement to purchase wetlands mitigation credits was included in the proposal and specifications. When contacting the approved bank, I was told that the price they provide is subject to change if a purchase agreement isn’t executed within 45 days.

As it is unclear what the duration will be prior to award, I ask that these items be converted to allowances rather than firm fixed price items.

Answer 1: Mitigation credit shall remain FIXED PRICE. Should the actual cost of the credits exceed the price quoted during bidding, the contractor may submit a request for an equitable adjustment for the cost difference. The contractor must be able to demonstrate through documentation that the quote received during the bid process was lower than the actual purchase price.

Question 2: Bid items 0002, 0019AB, 0019AJ, 0020AB, and 0020AK (the Floodwall and Culvert items) all include excavation and backfill as part of the scope of work. Where is the cost for the disposal of excess satisfactory material to be paid?

Answer 2: Excavated material is only to be hauled from the site if it is unusable for the rest of construction. If hauled from the site, cost will be paid under Bid Item 0011.

Question 3: In Amendment 2 the description for item 0011 Haul and Dispose states, "not including any work in Bid Item 0009." Is the reference to item 0009 correct? Should it instead be item 0020 (18AA)?

Answer 3: The reference to Bid Item 0009 should be to Bid Item 0014.

Question 4: There is over excavation and soil replacement shown on plan pages CG-104 and CG-105 and cross sections C-305 and C-306. This is mostly within the Option 1 limits of construction. Please confirm that the over excavation in this area is incidental Bid Item 0019AB and that the hauling and disposal of the over-excavated material for both the Base Bid and Option 1 is paid under item 0011 or 0014 depending on the characterization of the material and that the replacement soil is paid under item 0012.

Answer 4: Note that the majority of this material is required to be excavated for installation of the floodwall. Excavation and fill is incidental to floodwall Bid Item 0019AB. The excavated material is only to be hauled from the site if it is unusable for the rest of construction. If hauled from the site, cost will be paid under Bid Items 0011 or 0014 depending on characterization.

Question 5: With regard to the Base Bid Work versus the Optional Work, please clarify the following:

a. Please clarify why the entire C2 Flood Wall is not to be included in the Base Contract Price, and why certain sections of the C2 Flood Wall are bid as Optional Work.

b. Please clarify why it may take the Government up to 395 Calendar Days at NTP to determine if Option #1or Option #2 will be awarded?

c. Is it possible that Option #1 could be awarded without Option #2, or vice versa?

d. Is the entire C2 Flood Wall Project entirely funded by the Government for the Base Bid and Optional?

e. Are all Construction Easements and Rights‐of‐Way secured for this project?

Answer 5: The project is divided among a base and two options due to real estate acquisition.

All real estate in the base contract will be available by contract award; real estate in options1 and 2 is not yet acquired but is expected to be available within the 395 day period listed. In addition, the base contract is currently funded. Funding for options 1 and 2 is expected within the 395 day period listed.

Question 6: Please clarify if there is a cut‐off date for all questions to be submitted by the bidders

Answer 6: The cut-off date for all questions is COB 09/17/18.

Question 7: With regard to the Sheet Pile Cut‐Off Wall, please clarify the following:

a. The Sheet Pile structural Notes on Dwg S‐101, as well as the Sheet Pile Specification Section 31 41 16, appear to be providing a choice that allows the Contractor to select either Vinyl Sheet Piles, FRP Sheet Piles, or Steel Sheet Piles for the construction of the Cut‐Off Wall. Is this interpretation correct?

b. If the above interpretation is correct, please clarify why this highly unusual approach is being used for this project.

c. If all three Sheet Pile products are allowed to be used, then does the Government believe that all three Sheet Pile products are equal in their strength and performance?

d. In our experience, Vinyl Sheet Piles or FRP Sheet Piles are typically used in areas with relatively soft, unconsolidated sandy soils such as along the New Jersey coastal areas. Why does the Government believe that Vinyl Sheet Piles or FRP Sheet Piles are appropriate for the Green Brook project which requires the Sheet Piles to be driven to refusal into the weathered shale?

e. Considering the driving conditions on the Green Brook C2 Project, wouldn’t it make more sense to specify Steel Sheet Piles as the only reliable material to be used for the Cut‐Off Wall?

Answer 7: Amendment 2 eliminates vinyl and FRP sheet pile and requires steel

Question 8: Please clarify the apparent inconsistent use of “ highly weather shale” versus “weathered shale” in the notes on the drawings that apply to the Sheet Pile Cut‐Off Wall Refusal.

Note #5 on Dwg S‐805 uses the term “highly weathered shale” to define the Sheet Pile Cut‐Off Wall Refusal. However, Note #4 on Dwg S‐103 to S‐114 appears to use “weathered shale” to define the Sheet Pile Cut‐Off Wall Refusal. Yet, the Boring Logs use the following terms to classify the shale on this project: Decomposed Shale, Decomposed & Highly Weathered Shale, Highly Weathered & Decomposed & Shale, and Highly Weathered Shale. Since the Boring Logs do not differentiate between Weathered Shale and Highly Weathered Shale, please clarify which layer of Shale correctly defines the Sheet Pile Cut‐Off Wall Refusal.

Answer 8: The terms "weathered shale" and "highly weathered shale" are used synonymously on the drawings. There are two lines that pertain to sheet pile installation on the drawings. The lower of the two lines, labeled "Top of Weathered Shale and PZ22 Sheet Pile Tip Elevation" is the minimum depth to which the sheets must be driven for seepage considerations. The line labeled "SPT Refusal" has been included to indicate at what elevation additional driving effort may be required to reach the required tip elevation. See spec section 31 41 16 Sheet Piling, paragraph 3.2.2.2.f. In the event that a sheet reaches the required tip elevation but does not refuse, driving shall then continue until refusal.

Question 9: Please clarify how the Contractor is to determine the depths of the Sheet Pile Cut‐ Off Wall, for bidding purposes and in the field. For example, Note #4 on Dwg’s S‐103 to S‐114 states that the Sheet Pile Cut‐Off Wall is to be embedded some undefined distance into the Weathered Shale to Refusal. However, other Notes on the same drawings point to a dashed line below the footing on the Wall C2 Profile and indicate that the dashed lines represents the Top of Highly Weathered Shale and the Tip of the Vinyl Sheet Pile Cut‐Off Wall, which implies that the Sheet Pile Cut‐Off will not be embedded into the Weathered Shale. Since the Notes are not consistent, please clarify the expected depth of the Sheet Pile Cut‐Off Wall

Answer 9: The terms "weathered shale" and "highly weathered shale" are used synonymously on the drawings. There are two lines that pertain to sheet pile installation on the drawings. The lower of the two lines, labeled "Top of Weathered Shale and PZ22 Sheet Pile Tip Elevation" is the minimum depth to which the sheets must be driven for seepage considerations. The line labeled "SPT Refusal" has been included to indicate at what elevation additional driving effort may be required to reach the required tip elevation. See spec section 31 41 16 Sheet Piling, paragraph 3.2.2.2.f. In the event that a sheet reaches the required tip elevation but does not refuse, driving shall then continue until refusal.

Question 10: Please clarify how Vinyl Sheet Piles or FRP Sheet Piles can be driven into the Weathered Shale or Highly Weathered Shale without damaging these types of Sheet Piles.

Answer 10: Vinyl and FRP sheet piling have been eliminated by Amendment 2.

Question 11: Please clarify the method to be used by the Contractor to determine when the Cut‐ Off Wall Sheet Piles hit Refusal.

Answer 11: See Amendment 2, spec section 31 41 16 - Sheet Piling, paragraph 3.2.2.2.e

Question 12: Currently, Note #5 on Dwg S‐805, as well as Note #4 on Dwg S‐103 to S‐114, instruct the Contractor to use an Alternate Cast‐In Place Concrete Cut‐off Wall in lieu of the Sheet Pile Wall when the depth from the bottom of Pile Cap to the Top of Weathered Shale is 4’‐ 0” of less. Since the Boring Logs do not define any of the Shale Layers as “Weathered Shale” please revise the language in these Notes to be consistent with the Shale Classification terms used on the Boring Logs.

Answer 12: Based on the exploratory borings, the tip elevations for sheet pile are more than 4' below the bottom of pile cap, thus the alternate cut-off wall detail is not expected to be used.

The detail is included to cover the possibility that actual subsurface strata deviate from what was found during exploratory boring.

Question 13: We do not understand why the C2 Wall Profile Drawings do not indicate the areas of the project where the Alternate Cast‐In‐Place Cut‐Off Wall is to be used based on the Boring Logs. This type of design preference or choice can be shown on the drawings and should not be determined in the field, since that approach is relying on a “trial & error” approach. For example, Dwg S‐104 & S‐105 clearly show areas where the Top of Highly Weathered Shale is 4 feet or less from the bottom of footing. Yet, the plan view of the C2 Wall on these same drawings shows the sheet pile symbol and the notes indicate that Vinyl Sheet Piles are to be used in these same areas where the Alternate Cut‐Off Wall should be used. Therefore, we request that the Profile Drawings be revised to clearly show where the Sheet Piles are to be used and where the Alternate Cut‐Off Wall is to be used.

Question 14: On several of the C2 Profile Dwg’s S‐103 to S‐114 there are two dashed lines shown below the bottom of the footing. On some Profile Views these two dashed lines converge, while other Profile Drawings show a variable depth between the two dashed lines. With regard to these two dashed lines, please clarify the following.

a. We do not find the term “SPT Refusal” shown on any of the Boring Logs and we see Blow Counts shown at all depths. Please clarify why SPT Refusal is not shown on the Boring Logs.

b. Please clarify what soil layer or soil condition is defined by the dashed line defined as “SPT Refusal”.

c. What is the relationship between the two dashed lines

d. Why isn’t there a more constant depth between the two dashed lines?

Answer 14: SPT Refusal is defined as 50 or more blows advancing less than 6". The geotechnical report uses the term to highlight the refusal elevation. Those elevations determine the lines shown on the drawings. See revised spec 3.2.2.2 Driving

Question 15: There are too many micropile bid items. The work cannot be broken down into as much detail as they are requesting as many of the items overlap. For example, casing is installed as you are drilling, we cannot offer two separate pay items that both include the associated labor equipment and material. Can you submit an RFI and propose that the Micropile Pay Items are changed so that there are only two items: (1) LF of fully installed permanently cased portion of pile and (2) LF of fully installed Rock Socket/Bond Length portion of pile which should also include temporarily casing the hole if needed?

Answer 15: Micropile bid items remains. The intent is to allow the USACE to have unit prices for all the components of the pile installation operation in the event that unknown subsurface conditions are found during construction and a change order becomes necessary. The unit prices will be in the contract.

Question 16: Contract Drawing EC-104 indicates a 'potential contractor staging and stockpile' area in the vicinity of Block 244, Lot 9. This area is partially located within the limits of the base bid and partially with the limits of option #1 for this contract. As the Corps may take up to 395 calendar days to choose to award Option #1 or not, the contractor's staging area has the potential to be severely limited throughout the bulk, if not the entirety, of the contract duration.

Furthermore, there is a house on the property and those are to be demolished under a separate contract, possibly further limiting our timely access to this area. Is another staging area available within the base bid project limits or in the surrounding area?

Answer 16: The houses are being demolished by change order to the current Segment B3 construction contract. Houses will be gone by January 1, 2019. There is not another staging area available within the base bid project limits or in the surrounding area. The Contractors are of course free to obtain their own staging areas.

Question: 17: We cannot find a Bid Bond Form in the Contract Booklet, and we could not find Section 00 70 00 – Contract Clauses that was referenced in the Bid Guarantee clause of Section 00 73 00 of the Special Contract Requirements. Please clarify if there is a Government Bid Bond Form that is to accompany the bidder’s proposal.

Question 18 a.

With regard to the Sampling and Environmental/Chemical Testing of Imported Soil specified in Section 2.1 of the Earthwork Specification Section 31 00 00, please clarify the following:

a. The first sentence describes a series of chemical tests that are to be performed by the Contractor for the approval of Offsite Soils. Please clarify if this Testing complies with all of the requirements established by the NJDEP for Residential Direct Contact Clean Soil.

The first sentence of Part 3.15 of the Earthwork Specification Section 31 00 00 conflicts with Part 3.2.3 of the Seeding Specification 32 92 19 regarding the required preparation depth of the compacted subgrade. Section 31 00 00 says 2” deep and Section 32 92 19 requires 12” deep. Please clarify which specification section governs.

Based on the information collected during Government’s Subsurface Exploration Program, please clarify whether or not the excavated soils from the project site are expected (for bidding purposes) to be suitable for the backfilling of the new Flood Wall, Culvert C1, and other items, especially considering that the backfill will be used only for the support of landscaped areas.

Answer 20

Satisfactory material for general backfilling (non-structural, non-embankment) is defined in specification section 31 00 00 Earthwork, paragraph 1.2.1. The boring logs in the drawing set indicate types of soils expected to be encountered. It is expected that some of the excavated material will be satisfactory though it must be segregated from unsuitable material. The Contractor shall sample and test material intended for backfilling in accordance with paragraph 3.16 Testing to determine that it is satisfactory.

Question 21

Specification Section 09 97 02 requires the painting/coating of the Steel Micro Pile Casings as well as Steel Sheet Piles that may be used for the Cut-Off Wall. The coating of these two items is unnecessary and increases the cost of the project without benefit to the project. Once installed, these two items will be completely buried. Typically we do not see Micro Pile Casings provided with a protective coating, and the same observation applies to typical Sheet Pile Cut-Off Walls. In addition, the coating is likely to be damaged during the installation of both items, which negates any potential benefit of providing the coatings. Therefore, we request a revision to the Specifications that deletes the coatings for the Micro Pile Casings and Steel Sheet Pile Cut-Off Walls.

b. Please clarify why this area is to be treated differently from all of the other areas within the project limits.

c. Specification Section 31 00 00 does not have specific section that is called Embankment Material. Please clarify the type of material that is to be used for the backfilling of this area.

Therefore, in order to correct this problem, please insert the word “uncontaminated,” in the first line, just before the word “unsatisfactory”.

However, this ambiguity needs to be clarified. Therefore, please revise the description of work under CLIN #0004 to make it perfectly clear that the Borrow Material must meet the specific requirements established by the NJDEP for New Jersey Residential Direct Contact Soil Clean-Up Criteria (NJAC 7:26D) as established by the NJDEP. Please clarify.

a. The description of work for CLIN #0006 does not mention Sampling and Testing, which needs to be done by someone in order to characterize the soil as being ID-27.

Please clarify this issue.

b. Please clarify how the ID-27 Contaminated Soil will be identified on this project for Off- Site Removal and Disposal.

c. Is it the intent to have all excavated soil Sampled and Tested before the excavated soil is re-used as backfill?

d. Since Sampling and Testing Services can get very expensive, and since it is not known at this time the extent to which the existing soils will need to be sampled and tested, we recommend that a new CLIN be inserted into the price proposal to provide a Fixed Allowance to cover the Sampling and Testing costs that may be required to characterize the excavated soils.

e. Has the Government performed any sampling and testing of the existing soils to determine if there may be any contamination in the soil?

Answer 26 The Government has sampled and tested soils in the general area and not found contamination. However, this sampling does not guarantee that there is no ID27 material. It is the Contractor's responsibility to sample and test soil to be transported offsite and to include the cost in the bid item.

Question 27

We found Temporary Sheet Pile Cofferdams on the two of the Erosion Control Drawings EC- 101 and EC104. No other drawings indicate the need for Cofferdams. This can be interpreted by bidders to mean that the contract only requires Temporary Cofferdams at those locations. Is this correct? Please clarify the Temporary Cofferdam requirements for bidding purposes.

Answer 27

The temporary cofferdams shown on the Erosion Control drawings are a condition of the Soil Erosion and Sediment Control Certification to be issued by the Freehold Soil Conservation District for this project. The Contractor’s means and methods of construction may require other temporary cofferdams.

Question 28

With regard to the Demolition and Removal (Site Clearing) work to be performed by others, vplease clarify the following:

a. Please confirm that all tree stumps will be removed and backfilled by the other Site

Clearing Contractor.

b. Please confirm that basements will be completely removed, backfilled and graded with imported fill material by the other Site Clearing Contractor.

c. Please confirm that all disturbed areas will be temporarily seeded and mulched to control soil erosion by the other Site Clearing Contractor.

Please clarify the responsibility of the Contractor and the Government with regard to the topsoiled and seeded areas that are located within the Flood Plain areas because these areas are at high risk of being damaged by flooding after these areas are seeded by the Contractor. The Government needs to recognize the special nature of this work. By the time the Flood Plain seeding work is performed these areas cannot be protected with temporary cofferdams, which means that the Contractor is completely at the mercy of the river. This means that recently seeded Flood Plain areas could be completely obliterated by flood waters, and this could happen multiple times, depending on when the Base Bid work is performed and when the Optional Work areas will be awarded. In the event that this type of flood damage occurs, please clarify what actions will be taken by the Government to make the Contractor whole. The bidders need to know if the Contractor will be compensated for replacing lost topsoil and for re-seeding the flood damaged Flood Plain areas.

Answer 29 The risk associated with damage to top soil, seed and plants shall be assumed by the Contractor. Please see specification section 32 92 19, paragraph 1.7.5: “Under the terms and conditions of the one-year plant guarantee – seeding, the Contractor shall be solely responsible for plant survival in seeded areas. Losses attributed to herbivores, disease, biodegradation, inappropriate hydrologic regimes, drought, insect damage, fire, wind or wave energies, ice scouring or vandalism shall not lower the minimum survival or coverage requirements.” Reference drawing VH601 includes a 20-year record of river discharges and stages to assist the bidders in assessing risk to build into their bids. Please note that the rating curve on drawing VH601 gives river stages in the NGVD datum while the project drawings use the NAVD88 datum.

Question 30 a.

With regard to the Painting of the Flood Wall, please clarify the following items in the Painting Specification Section 09 97 02.

a. A review of the Painting Specification Sections 1.3.1.4, 1.3.1.5, 1.3.1.6, 1.3.1.7, 1.3.2, indicates that this is a generic specification that applies to primarily to painting that is performed in a confined space within an enclosed building, such as a Paint Shop, and is primarily focused on painting metal surfaces. Therefore, please revise, remove or clarify the numerous conflicting requirements in the Painting Specifications that are not applicable to the Painting of the Flood Wall, which is not metal and will not be performed in a Paint Shop. These revisions are needed to avoid unnecessary confusion during the bidding phase of this project as well as during the construction phase of this project.

Answer 30 a.

Please be aware that the sheet piling – which is now required to be steel as part of Amendment 2 – and the micropile casings are to be coated with coal tar epoxy which is assumed to happen in an enclosed environment. The specification paragraphs cited in the question apply to painting of these items and not to painting of the floodwall.

b. Please clarify the intent of Painting Section 1.5.1 which requires the submittal of the “certificate for each Certified EHS Professional” who will provide “preconstruction and on-site environmental, safety and health services”. This section does not appear to pertain to the Painting of the Flood Wall. Please either remove Section 1.5.1 or provide clarification regarding the relevance of Section 1.5.1 to the Painting Specifications.

c. Section 3.5 is either organized incorrectly or it is missing words, which is making it difficult to interpret with any certainty. Please read the spec section carefully. The micropile casings and steel sheet pile are items a and b respectively in paint System 6 (coal tar epoxy); the floodwall is item a – the only item – under paint System 25 (concrete coating system).

d. The coatings that are specified in Section 3.5 for the Flood Wall only specify products produced by Euclid Chemical. However, there should be other coating manufacturers that produce coating products that are considered equal to the specified Euclid Chemical coatings. Please provide additional coating suppliers and coating products that are considered “Equal”.

Please see revised spec section 2.2.2 Concrete Floodwall Paint. This specification is the only coatings approved for use on this project. They match the colors and coating characteristics of floodwalls previously constructed and to be constructed in the future on the Green Brook project.

Please clarify the size of the portion of Culvert C1 that is to be constructed in Option #2.

Due to some conflicting details and notes, we are not certain if the Culvert in Option #2 is to be 6’ x 2.5’ or 6’ x 3’, or both.

Please see drawing S-113, the Plan – Grate Inlet C1-2 on S-701 and Section E on S-

702. These make clear that the inside height of the culvert increases from 2’-6” to 3’-0” very near to inlet C1-2 which is well within the limits of Contract Option 2. Therefore, the culvert is both 2’-6” high and 3’-0” high within the limits of Contract Option 2. This is also apparent on drawing C-201. (Please notice that the culvert profile on C-201 is drawn in reverse direction to the civil and structural plan views – the upstream or higher elevation of the culvert invert is on the left on C-201 but on the right side of the civil and structural plan drawings.)

SECTION 00 73 00

SPECIAL CONTRACT REQUIREMENTS

1. COMMENCEMENT, PROSECUTION AND COMPLETION OF WORK

1. COMMENCEMENT, PROSECUTION AND COMPLETION OF WORK

a. The Contractor shall be required to (i) commence work under this contract within 5 calendar days after the date the Contractor receives the notice to proceed, (ii) prosecute the work diligently, and

(iii) complete the entire work ready for use not later than 540 calendar days after the date the Contractor receives the notice to proceed. The time stated for completion shall include final cleanup of the premises except in case the Contracting Officer determines that seeding and exterior planting is not feasible during the construction period (FAR 52.211-10). The Contractor shall accomplish such seeding and exterior planting in the first planting period as specified in Paragraph 3.1.1 of Section 32 92 19 following the contract completion date. This action will not operate to extend the performance time for the balance of the work.

Contract Option may be exercised at the discretion of the Government and are not mandatory.

Contract option 1 may be awarded within 395 calendar days from receipt of NTP, if Option item 1 is awarded 180 calendar days will be added to the period of performance

Contract option 2 may be awarded within 395 calendar days from receipt of NTP, if Option item 2 is awarded 180 calendar days will be added to the period of performance

If Contract Option 1 is exercised , CLIN 0019 (0019AA thru 0019AK) shall be exercised If Contract Option 2 is exercised , CLIN 0020 (0020AA thru 0020AK) shall be exercised

b. Location: The site of work is located in Middlesex, NJ, along the right bank of Bound Brook (creek), east and west of South Lincoln Avenue.

c. The Contractor shall furnish all labor, materials, equipment, and services (except those furnished by the Government) for the following work: Construction of concrete floodwall supported on drilled micropiles, construction of concrete culverts and drainage features, associated earthwork and seeding.

d. All work shall be in accordance with the drawings and specifications or instructions attached hereto and made a part thereof, or to be furnished hereafter by the Contracting Officer and subject, in every detail, to his supervision, direction, and instructions.

e. Magnitude of Construction Project: The estimated value of the proposed work is between $10,000,000 and $25,000,000

2. LIQUIDATED DAMAGES – CONSTRUCTION

a. If the Contractor fails to complete the work within the time specified in the contract, or any extension, the Contractor shall pay to the Government as liquidated damages, the sum of $2,219 for each day of delay.

b. If the Government terminates the Contractor’s right to proceed, the resulting damage will consist of liquidated damages until such reasonable time as may be required for final completion of the work together with any increased costs occasioned the Government in completing the work.

c. If the Government does not terminate the Contractor’s right to proceed, the resulting damage will consist of liquidated damages until the work is completed or accepted.

d. At a time before the project is physically complete but is functionally complete to the satisfaction of the Government, the Government at its sole discretion may agree to accept transfer of the facility or project provided that the remaining work to be done ("punchlist") is completed no later than 30 days from the date of transfer. In this case the contractor shall pay liquidated damages for punchlist items not completed in the daily amount of $555.00 per day commencing after 30 days of project transfer or after date required for project completion (including all extensions), whichever occurs later.

3. EQUAL OPPORTUNITY PREAWARD CLEARANCE OF SUBCONTRACTORS

Notwithstanding the clause of this contract entitled “Subcontracts”, the Contractor shall not enter into a first-tier subcontract for an estimated or actual amount of $1 million or more without obtaining in writing from the Contracting Officer a clearance that the proposed subcontractor is in compliance with the equal opportunity requirements and therefore is eligible for award.

4. CERTIFICATES OF COMPLIANCE

Any certificates required for demonstrating proof of compliance of materials with specification requirements shall be executed in 4 copies. Each certificate shall be signed by an official authorized to certify on behalf of the manufacturing company and shall contain the name and address of the Contractor, the project name and location, and the quantity and date or dates of shipment or delivery to which the certification applies. Copies of laboratory tests reports submitted with certificates shall contain the name and address of the testing laboratory and the date or dates of the tests to which the report applies. Certification shall not be construed as relieving the Contractor from furnishing satisfying material, if, after tests are performed on selected samples, the material is found not to meet the specific requirements.
