1. This clearance authorizes commencement of excavation work only and is not a notice to proceed or work authorization.
2. This excavation clearance is effective upon the date of signature by DPW official below; EXCAVATION SHALL COMMENCE WITHIN 30 CALENDAR DAYS.
3. Requester will provide two (2) copies of required work site drawings with excavation clearance request. DPW will retain one (1) copy of these drawings.
4 Requester will retain original completed excavation clearance request onsite during excavation activities. DPW will retain one ( 1) copy of the request.
5. Boring locations must be marked on a 1/100 scale drawing.
6. All soil to be removed from the work site shall be disposed of in accordance with guidance from the DPW Environmental Resources Department.
7. An after action report (AAR) concerning work at sites where institutional controls are in place shall be submitted to DPW Environmental Resources Department NL T 30 days follow ing completion of work.

Ft. Wainwright Excavation Clearance Request (Dig Permit) (Updated as 16 October 2009)
1. Prior to the commencement of any excavation, trenching or other digging activity for all soil disturbing activities impacting soils six inches or more below the ground surface, the contractor shall obtain an Excavation Clearance Request (ECR), USARAK Form 81-E. USARAK Form 81-E is commonly referred to as a "Dig Permit". The Contractor shall be responsible for providing all information required by the ECR to include obtaining all signatures from the organizations and individuals required on the ECR. A blank ECR form may be obtained from the Customer Service Desk at the Directorate of Public Works, Building 3015, Fort Wainwright.
2. If contaminated soils, drums, unexploded ordnance, or unusual debris are found on or around any work site, the contractor shall stop work immediately and notify the Fire Department or "9 11 ,. in accordance with USARAK Pamphlet 200-1 , Hazardous Materials and Regulated Waste Management. Additionally, the contractor shall notify the Contracting Officers Representative (COR) immediately of any such action. Work at the site will be suspended until the area is cleared by DPW Environmental Resources and notification by the COR is given to the contractor allowing work to continue. Site clearance by Range Control is required if unexploded ordinance is involved.
3. Contaminated soil or groundwater removed from the work site must meet container type, sampling and analysis for potential contamination, marking and labeling, and moving and storage requirements specified in Pamphlet 200-1 or as otherwise specified by DPW Environmental Resources prior to removal. Soil and groundwater shall not be removed from any part of the installation without written authorization from an authorized USARAK representative.
All operations involving hazardous waste will be accomplished in accordance with USARAK Regulation 200-4, Environmental Quality: Hazardous Waste, Used Oil and Hazardous Material Management and USARAK Pamphlet 200-1 , Hazardous Material and Regulated Waste Management.

CHECKLIST
OPEN FLAME, CUTTING, WELDING & SOLDERING
This checklist is required for all welding/open flame and cutting operations outside of an authorized shop. Prior to starting please ensure that all information on your permit is filled out completely and correctly. The individual responsible for the operation and the Fire inspector must sign the permit. By following the guidelines below will ensure you have a safe operation.
1. FIRE/EMERGENCY REPORTING PROCEDURES
a. Stay calm and evacuate the building.
b. Dial 911. If safe to do so attempt to extinguish the fire.
c. Give building number and specific area of the fire.
d. Give your name and callback number. Do not hang up until told to do so by the Fire
Department Dispatcher.
e. Report ALL FIRES regardless of size even if they have been extinguished.
2. CHECK YOUR EQUIPMENT
a. Ensure your equipment is in good working order.
b. Do you have personal safety equipment (hood, aprons, gloves, etc).
c. Do you have your own Fire Extinguisher? Water/ABC Dry Chemical. You cannot use the buildings fire extinguisher.
d. Do you have Noncombustible Covers? All combustible materials must be covered within a 35 food radius if you’re unable to move them out of the hazard area.
3. SAFETY PROCEDURES
a. Ensure no Flammable/Combustible liquids or vapors are present.
b. Locate Oxygen/Acetylene cylinders as far from the hazard area as possible
c. No Smoking in the hazard area or within 50’ of gas cylinders
d. Wetdown may be required in some areas prior to operation
e. Close cylinder valves when not in use. Drain regulators and hoses of any pressure when operation is complete.
f. Notify the Fire Dept at 353-6485/7470/6546 for inspection of the area upon completion of operation, prior to closing the building.
g. You as the operator, is responsible for complying will all safety regulations and safety of the work site at all times.
4. PERMIT
a. DO NOT Start work prior to obtaining a permit from the Fire Dept.
b. Permit must be maintained on work site or where the operation is being performed.
c. Permits are only valid for one time and one location unless arrangements have been made and noted on the permit from the Fire Inspector issued the permit.
d. If you fail comply with any Fire/Life/Environmental safety requirements stated, your permit will be revoked.
