PAST PERFORMANCE QUESTIONNAIRE
I. Background
The purpose of this questionnaire is to obtain past performance information relative to contracts awarded in the past three years which are of similar scope, magnitude, and complexity to our current requirement. This company is a potential source for performing a Firm Fixed Priced contract. Your assistance in evaluating the contractor’s past performance is deeply appreciated.
II. Instructions
Please complete the questionnaire based on the following guidance:
Responses may be typed or hand written. Indicate based on the description below, the contractor’s performance on the identified program in the assessment area. The following is a definition of the scoring levels:

2. The Contractor hired quality Subcontractors and effectively managed multiple Subcontractors to support high task order volumes (100 or more).
3. The Contractor developed realistic project schedules and completed projects within established schedules.
4. The Contractor has performed the contract requirements at the original awarded price without sacrificing quality, cutting corners, or complaining about low profit margins.
5. The on-site Project Manager was given sufficient decision making authority and has been able to authorize solutions for any technical, management, or management support requirements that have been encountered.
6. The Contractor has paid Subcontractors timely and without labor violations or claims.
7. The Contractor has completed task orders requiring compressed project schedules on time and without adversely affecting other in-progress projects.
8. The Contractor has provided adequate information and pricing for change orders and has negotiated prices for change orders that were fair and reasonable.
9. The Contractor has provided timely and accurate progress schedules with invoices.
10. Deficiencies and punch-list items have been corrected in a timely manner.
