Represent all final monetary extensions in two decimal positions only. Example:

b. Unit prices and extended prices cannot be left blank; indicate your intent to not charge a specific CLIN or
SubCLIN by writing the unit price as $0.00 and total/estimated amount price as $0.00.
c. Offerors are not required to propose prices for CLINs 0005, 0006, 0007, 1005, 1006, 1007, 2005, 2006, 2007, 3005, 3006, 3007, 4005, 4006, and 4007. CLIN 0005 and associated option year CLINs will be priced per negotiated Task Order. The Loaded Labor Rate Table will be used as the basis for fully loaded labor rates of proposed personnel to complete work associated with these CLINs.
d. The Government requires the submission of Other Than Certified Cost or Pricing Data as it is deemed necessary to arrive at a fair and reasonable price. Offerors can submit its data on the provided Excel worksheet titled “Blank Pricing Worksheet” (Section J, Attachment 24) or its own worksheet as long as it provides the same level of detailed information to support its offered prices.
e. Regardless of the format for submitting the Other Than Certified Cost or Pricing Date, offerors shall complete and submit the Loaded Labor Rate Table (Section J, Attachment 25) in support of its price proposal.
f. The offeror shall submit catalogs, price lists, and/or other data that demonstrate that the prices offered for this contract (including underlying labor rates, materials costs), are fair and reasonable, and reflect pricing that is comparable to that which is charged to other customers for same or similar requirements. Offeror is reminded to include all costs (i.e., labor, travel, overhead, General and Administrative (G&A), profit) associated with the performance of the requirements. Submit supporting data to explain how proposed price for CLINs were estimated in sufficient detail to enable a complete and auditable understanding of the basis of the prices associated with the performance of all work under these CLINs.
4. Volume IV - Past Performance. The Past Performance evaluation will be accomplished by reviewing aspects of an Offeror's recent and relevant Past Performance for a minimum of three and a maximum of five contract references, focusing on and targeting performance that is relevant to the effort.
a. Past Performance information described herein is required about the offeror and all subcontractors, teaming partners, and/or joint venture partners proposed to perform 50% of the proposed effort based on the total proposed price. The offeror shall submit, along with the information required in this paragraph, a consent letter, executed by each subcontractor, teaming partner, and/or joint venture partner, authorizing release of adverse past performance information to the offeror so the offeror can respond to such information. For each identified effort for a commercial customer, the offeror shall also submit a client authorization letter, authorizing release to the Government of requested information on the offeror's performance.
b. The offeror shall include documentation regarding its relevant past performance as it directly relates to the work being procured under this solicitation. The offeror SHALL NOT go back any farther than 3 years from the proposal submission date for services for the submitted data. The past performance data shall document a successful history of past contract performance. Past performance information submitted by the offeror older than 3 years will not be evaluated.
c. In conducting the past performance evaluation, the Government reserves the right to use both the information provided in the offeror’s Past Performance Volume and information obtained from other sources, such as the Past Performance Information Retrieval System (PPIRS) or similar systems, Defense Contract Management Agency (DCMA) and commercial sources. Offerors are reminded that both independent data and data provided by offerors in their proposals may be used by the Government to evaluate offeror past performance. However, the burden of providing thorough, complete, and current past performance information as requested in this paragraph remains with the offerors. Proposals that do not contain the information requested by this paragraph risk rejection or a less than acceptable performance rating by the Government. All past performance comments received will be taken into account and could affect the overall rating. The overall past performance evaluation is a subjective decision based on the whole of all data received. In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favourably or unfavourably on past performance (see FAR 15.305 (a)(2)(iv)). Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “Acceptable.”
d. Submission Requirements. The offeror shall submit a Past Performance Volume containing the following:
Summary Page describing the role of the offeror and each subcontractor, teaming partner, and /or joint venture partner that the offeror is required to provide past performance information in accordance with Section L, Paragraph D. 4. a.
Consent Letter executed by each subcontractor, teaming partner, and/or joint venture partner, authorizing the release of past performance information so the offeror can respond to such information.
A sample consent letter is attached as Attachment 1.
Client Authorization Letter for each identified effort for a commercial customer authorizing release to the Government of requested information on the offeror’s performance.
Organization Structure Change History. Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, and subsidiary companies. In many cases, these changes have taken place during the time of performance of relevant present or past efforts or between conclusion of recent past efforts and this source selection.
As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition.
To facilitate this relevancy determination, include in this proposal volume a "roadmap" describing all such changes in the organization of your company. As part of this explanation, show how these changes impact the relevance of any efforts you identify for past performance evaluation/performance confidence assessment. Since the Government intends to consider present and past performance information provided by other sources as well as that provided by the offeror(s), your "roadmap" should be both specifically applicable to the efforts you identify and general enough to apply to efforts on which the Government receives information from other sources.
Specific Relevant Contract References. The offeror shall submit the Recent and Relevant Past
Performance Contract Reference Sheet, Attachment 2, outlining the offeror’s past performance with contracts, as a prime or major subcontractor, which is the same or similar in nature, size, and complexity to the services being procured under this Solicitation. The submittal shall include rationale supporting your assertion of relevance and how it was determined that the work performed previously was the same or similar in nature, size, and complexity to the work specified by this solicitation.
Offerors are required to explain what aspects of the contracts are deemed relevant to the proposed effort, and to what aspects of the proposed effort they relate. The offeror shall submit a minimum of three (3) and a maximum of five (5) contracts that demonstrate the ability to handle a contract of the size, scope, and complexity of this requirement performed within the past 3 years. Entities that do not have any recent or relevant past performance references shall annotate so in its submission.
Past Performance Questionnaire. For each reference provided, the offeror shall forward a copy of the
Past Performance Questionnaire located in Attachment 3 to the point of contact responsible for monitoring performance under such contracts. The point of contact shall return the completed questionnaires directly to the contract specialist, Wendell Fulcher, via email to wendell.g.fulcher.civ@mail.mil, by 2:00 PM PDT, 23 November 2018. Any questionnaires not returned directly to the contract specialist will not be reviewed and evaluated. New entities that have no past customers shall annotate on the Summary Page that they have had no previous clients and that the minimum number of questionnaires cannot be provided.
