1. I, _____________________________ have been issued a Cellular Phone and/or Blackberry device as described on the attached page for the conduct of my mission, and with my signature below, indicate that I have read and understand USAOTC Policy Memorandum G6-1 concerning the use of the device(s).

2. I understand that the phone device has a ________ minute plan and I will not exceed these minutes. Minutes used can be checked using the cellular phone features; see user’s manual for the model issued.

3. I will use the wireless device(s) (including cell phones, blackberries, aircards) for official business only, except as allowed in AR 25-1 and DOD Joint Travel Regulation.

4. This phone will be limited to requirements that cannot be satisfied by other available telecommunication methods (land lines) or email.

5. I will not discuss classified information over the device or unclassified information that may result in a compromise of Operational Security.

6. I understand that I am not authorized to use the cellular phone in geographic areas outside of the area of service coverage and where usage incurs roaming charges. If in doubt, I will check with G6, Wireless Telephone Control Officer (WTCO), before making any phone calls.

7. I understand that I can only place calls to phone numbers within the continental United States. I further understand that I am not authorized to call any numbers outside the continental United States unless international service has been coordinated with G6, WTCO prior to going TDY.

8. I will not attempt to modify the service on my wireless device.

9. I understand that I am not authorized to access, purchase, or download any multimedia, videos, ring tones, internet packets, subscriptions or anything else that incurs any type of cost to the government.

10. I understand that I am not authorized to send or receive any text messages unless I have a pre-authorized monthly texting plan added to my service.

11. I understand that I will not forward office and residence phone calls to government cellular phone numbers, nor forward government cellular phone calls to office or residence phone numbers.

12. I will not use a cell phone while operating a vehicle on a military installation unless a hands-free device is used (exceptions include emergency responders).

13. I understand I am prohibited from using Bluetooth headsets or Bluetooth hands-free devices with any type of DoD cellular or Blackberry device.

14. I will reimburse the government for the repair or replacement cost of a damaged, lost, or stolen device, if its damage, loss, or theft was due to my negligence.

15. Use of the DOD information system indicates that the user consents to monitoring and understands that the command or agency has a right to inspect and audit all information. I understand that IAW AR 25-1, that all information systems, which include cellular phones and communications made using the device, are subject to monitoring. During monitoring, information may be examined, recorded, copied, and used for authorized purposes. All information, including personal information placed on or sent over DOD computer systems, may be monitored.

16. I understand that the G6, WTCO, audits usage of the device on a monthly basis.

17. I acknowledge that I will be held financially responsible for any and all charges incurred that are not in accordance with this agreement. I will reimburse the government for the charges due to unauthorized use of the device.

18. I will immediately report lost or stolen wireless devices to my Directorate TCO or Test Officer and make sure G6, WTCO is notified.

19. I will return the device to the Directorate TCO or the Test Officer for turn-in when it is no longer required to carry out my work assignment or upon change of work assignment.

20. Within 14 days of receiving the device I will complete wireless training and provide a copy of the certificate to the Directorate TCO or the Test Officer. Training is available at -http://iase.disa.mil/eta/pedrm_v2/pedrm_v2/launchPage.htm
