CONTRACTOR PERFORMANCE STATUS REPORT

The CPSR shall consist of six (6) tabs labeled: Task Order, Worker, Assignment, Status of Task Order Packages, Contractor Monthly Reviews of Quality Control Plans & Government Actions. The CPSR must be submitted no later than the 10th calendar day following the end of the month. The CPSR is a dynamic document, that is continually edited and updated to meet all the requirements in the PWS and contract and task orders. Description of each tab is outlined below:
TAB 1: Task Order - This tab is designed to be imported into a Government surveillance application. It provides the task orders that were awarded to the Contractor during the reporting period. Information shall include but not limited to: Task Order Number, Task Order Award Date, CLIN, Labor Category and Position Title, Number of Identifying Position Number, Ordering Period, Hourly Rate, Personal or Non- Personal Services, POP, Start Date, Performance Location. Ordering Office and POC
TAB 2: Worker - This tab is designed to be imported into a Government surveillance application. It provides the list of HCW who are employed by the Contractor and will be performing on this contract/task orders.
TAB 3: Assignment - This tab is designed to be imported into a Government surveillance application. It provides the assignment of a HCW to a specific task order and position at a specific level of effort or full time equivalent (FTE).
Each HCW assignment, either initial or replacement, must be reported on this tab. A replacement assignment can only occur after the assignment of the replaced HCW has ended (i.e., John, whose level of effort is 1 FTE, can only be listed as a replacement for Joe, whose level of effort is also 1 FTE, when Joe's assignment has ended. This requires an entry for Joe with an end date of his assignment). The total number of active assigned FTEs on a labor category cannot exceed the required number of FTEs for the labor category.
TAB 4: Status of Task Order Packages
TAB: Contractor Quality Control Plan – Monthly Review/ Status
TAB 5: Government Actions - This tab is designed for the Contractor to report Government action during the month. The columns are available for reporting on each Government action.
