About this file

This is a Past Performance Assessment Questionnaire template used to evaluate contractor performance for a Family Advocacy Program (FAP) Victim Advocacy Program (VAP) and Emergency Placement Care (EPC) Support Services contract opportunity from the Army Contracting Command. The questionnaire is to be completed by points of contact familiar with the contractor's past performance and returned to Contracting Officer Carlos Mayorga and Contract Specialist Jessica Spencer.

The questionnaire contains rating definitions ranging from "Substantial Confidence" (exceeds many requirements) to "No Confidence" (does not meet most requirements) and evaluates contractors across multiple areas: compliance with specifications and workmanship standards, effectiveness of project management, timeliness of deliverables, and commitment to customer satisfaction. Each rating requires a detailed rationale. The form includes sections for contract identification details, offeror representative information, and respondent identification. This is purely an evaluation template and does not contain any specific performance information or completed evaluations.
