SECTION L

INSTRUCTIONS TO OFFERORS

FOR

UNITED STATES AIR FORCES EUROPE (USAFE) – AIR FORCE AFRICA (AFAFRICA)

ADVISORY AND ASSISTANCE SERVICES (A&AS) V

13 July 2022

REQUEST FOR PROPOSAL FA564122R0004

1. GENERAL INSTRUCTIONS

The is a best value source selection conducted in accordance with Federal Acquisition Regulation (FAR) 15.3 as supplemented.

1.1. Proposal Compliance. To ensure timely and equitable evaluation of proposals, and the Offeror's proposal shall include all data and information requested by these Instructions to Offerors and shall be submitted in accordance with these instructions. The proposal shall be compliant with the requirements as stated in the solicitation and the Performance Work Statement (PWS), Solicitation Attachment 11 for the Indefinite Delivery Indefinite Quantity (IDIQ) Contract, and all other parts of this solicitation. If a proposal fails to conform to the instructions provided in this instruction, the Government may deem it non-responsive and the Offeror may be deemed ineligible for award. Offerors are NOT permitted to take exception to any terms and conditions within the Request for Proposal (RFP).

1.2. Proposal Clarity. The proposal shall be clear and concise, and shall include sufficient detail for effective evaluation and for substantiating the validity of the claimed points on the Self-Scoring Matrix (Solicitation Attachment 12). Offerors shall assume that the Government has no prior knowledge of their experience and will base its evaluation on the information presented in the Offeror's proposal.

Offerors shall not submit elaborate brochures or documentation, detailed artwork, or other embellishments.

1.3. RFP Discrepancies/Ambiguities. If an Offeror believes that any portion of this solicitation contains a perceived error, omission, or is otherwise unclear, the Offeror shall immediately contact the Procuring Contracting Officer (PCO) and Contract Specialist via e-mail, clearly describing the portion of the solicitation in question and the impact the discrepancy or ambiguity has on the Offeror’s ability to prepare its proposal. Questions pertaining to the proposal shall be submitted no later than the date and time identified in Paragraph 1.4 below.

1.4. Point of Contact. The PCO is the sole Government point of contact for this acquisition. Offerors shall address any questions and concerns related to this solicitation to the PCO as soon as possible, but no later than 1500 Central European Summer Time (CEST), 26 August 2022. The subject of the email shall be “Questions – FA564122R0004”. Questions shall include a reference to the specific solicitation element to which they pertain. The Government will take into consideration all questions submitted by the deadline set forth above and will answer them via posting to SAM.gov. Written requests for clarification and questions shall be sent to the acquisition mailbox at 764ESS.PK.AAS-V@us.af.mil with a copy to the PCO, Capt Teresa Elsbree at teresa.elsbree.2@us.af.mil, and to the contract specialist Ms. Martina Honneffer at martina.honneffer.de@us.af.mil.

1.5. Communications. All exchanges of source selection information between the Government and the Offerors will be controlled by the PCO. Source selection information will be transmitted to Offerors electronically, e.g., via email, or the Solicitation Module within the Procurement Integrated Enterprise Environment (https://piee.eb.mil/). If transmitted via email, the transmission shall be sent and received encrypted and must include “Source Selection Information – See FAR 2.101 & 3.104” in the Subject line.

1.6. Electronic Reference Documents. All referenced documents for this solicitation can be found through the solicitation announcement at SAM.gov, the authoritative source for Government Contract Opportunities. The solicitation notice at SAM.gov redirects visitors to the Solicitation Module within the Procurement Integrated Enterprise Environment (https://piee.eb.mil/), where all solicitation details and associated documents can be found. Offerors are encouraged to subscribe for real-time email notifications for when information has been posted to the website for this solicitation. Offerors may contact the PCO if they are unable to locate any of the referenced material.

1.7. Proposal Records. In accordance with FAR Subpart 4.8 (Government Contract Files), the Government will retain one copy of each Offeror’s proposal.

1.8. Debriefings. The PCO will promptly notify Offerors of any decision to exclude them from the source selection; whereupon, the Offeror may request and receive a debriefing in accordance with FAR 15.505. Offerors desiring a debriefing shall make their request in accordance with the requirements of FAR 15.505 or 15.506, as applicable. Offerors are entitled to no more than one debriefing.

1.9. Page Format

1.9.1. Page Limitations. Page limitations shall be treated as maximums. If exceeded, the Government will not read or consider the excess pages in the evaluation of the proposal. Each page shall be counted except the following: cover pages; table of contents and indexes; list of figures, or drawings; tabs; black pages; glossaries; cross reference matrices; and document revision history tracking records. Pages that do not count towards the page limitation shall be numbered in a manner that clearly identifies the page as a page that does not count toward the page limitation. The Government may place page limitations on responses to Evaluation Notices (EN), if ENs are determined necessary.

1.9.2. Page Size and Format

General Page Instructions. A page is defined as each face of a sheet of paper containing information. Page size shall be 8.5 x 11 inches unless stated otherwise within this document. Pages shall be single spaced, and the text size shall be no less than 11 point Times New Roman or Arial Font unless stated otherwise within this document. All proposal text shall be written in English. Offerors shall use at least 1 inch margins on the top and bottom and ¾-inch side margins unless stated otherwise within this document. Where Government templates are utilized (Work Sample Cover Sheet, Past Performance Questionnaire, etc.) Offerors shall keep margins, page sizes, and page orientation identified in the template. Pages shall be numbered sequentially by volume, with each volume starting at page one. These page size and format restrictions shall apply to responses to ENs if exchanges with Offerors are determined necessary.

1.10. Table and Chart Instructions. Legible tables and charts may be used. These displays shall be uncomplicated, legible, and shall not exceed 11 x 17 inches in size. The 11 x 17 size may only be used for large tables and charts; they shall not be used for pages of text. For tables and charts, the text shall be no less than 10 point Times New Roman Font, and margins shall be no less than ½ inch on all sides unless stated otherwise within this document. An 11 x 17 inch page, as authorized under this section, will count as one page.

1.11. Electronic Submission Format. The Offeror shall submit each volume in electronic format. The proposal shall be submitted in a format readable by Adobe PDF and MS Office Excel (where applicable). Offerors shall use separate electronic files for each proposal section listed in Table 1 to permit rapid location of all portions of the proposal. Exhibits, annexes, and attachments which are provided in support of a section shall be included in the electronic file for that section. Offerors shall provide each work sample as a single separate electronic file. If files are compressed, the necessary decompression program shall be included.

1.12. Proposal Markings. Proposals shall be marked in accordance with FAR 52.215-1(e), Restriction on Disclosure and Use of Data, and FAR 3.104-4, Disclosure, Protection, and Marking of Contractor Bid or Proposal Information and Source Selection Information. Offerors shall not include proprietary markings on attachments and documents that will become a part of the contract.

1.13. Cross-Referencing. To the greatest extent possible, each volume shall be written on a stand-alone basis so that its contents may be evaluated with no cross referencing to other volumes of the proposal. Information required for proposal evaluation which is not found in its designated volume and/or section will be assumed to have been omitted from the proposal. Cross-referencing within a proposal volume is permitted where its use would conserve space without impairing clarity.

1.14. Table of Contents. Each volume shall contain a detailed table of contents to delineate the subparagraphs within that volume. However, a table of contents incorporating the details of Work Sample pages already outlined in Solicitation Attachment 13, Cross-Reference Matrix is not required. Tables of contents do not count against the page limitations for their respective volumes.

1.15. Glossary of Abbreviations and Acronyms. Each volume shall contain a glossary of all abbreviations and acronyms used, and with an explanation for each. Glossaries do not count against the page limitations for their respective volumes.

1.16. Teaming Arrangements. Offerors submitting a proposal as the prime Offeror in response to this solicitation are permitted to submit a separate proposal under which they would be a Subcontractor, or team member to another prime contractor. Subcontractors are permitted to support multiple prime contractors.

1.17. Ordering Procedures. The Government intends to issue multiple-award indefinite-delivery, indefinite-quantity contracts. The Post-Award Conference Task Order will fulfill the minimum order requirement. Future ordering against A&AS V will be conducted in accordance with the Fair Opportunity Ordering Procedures listed in FAR part 16.505 Ordering.

2. PROPOSAL INSTRUCTIONS

2.1. Proposal Due Date. The proposal due date and time for this solicitation is 11 October 2022 at 1500 Central European Summer Time (CEST). Proposals received after this time will be considered late. Late proposals will not be evaluated or considered for award. Proposal submission for this solicitation will be electronic via the Solicitation Module within the Procurement Integrated Enterprise Environment (https://piee.eb.mil/); no paper copies are required or will be accepted. Upon submission of their proposal in PIEE, an Offeror shall provide confirmation (screenshot or PDF of the PIEE documentation) via email to the acquisition mailbox at 764ESS.PK.AAS-V@us.af.mil with a copy to the PCO, Capt Teresa Elsbree at teresa.elsbree.2@us.af.mil. Confirmation shall include a date/time stamp documenting submission of the proposal in PIEE before the proposal due date.

2.2. Proposal Organization. The Offeror shall prepare the proposal as set forth in the Table 1, Proposal Organization. Only one (1) digital media copy of the proposal is requested. The volume folders and section files shall be as defined in this table, all of which shall be within the required page limits specified in the Table 1, below.

3. VOLUME I - EXECUTIVE SUMMARY

3.1. Organization. This volume shall be organized according to Table 1 above.

3.2. Offeror Company Information/Cover Letter. The Offeror shall provide a Cover Letter with all of the following company information: Name, Address, Cage Code, Unique Entity Identification Number (UEI), System for Award Management (SAM.gov) registration date, proposal validity statement, and Contracting and Technical POCs (name, email address, and phone number). The proposal validity statement shall be valid through 270 calendar days from the proposal due date. The Offeror shall be registered in the SAM.gov at the time of proposal submission, through contract award and for the duration of the acquisition. If the Offeror’s SAM.gov registration expires prior to the proposal validity date, the Offer shall assert its intent to renew the registration in the Executive Summary.

3.3. Authorized Offeror Personnel. The Offeror shall provide the name, title, email address and telephone number of the company/division POC regarding decisions made with respect to its proposal and who can obligate the company contractually. Also, the Offeror shall identify those individuals authorized to communicate with the Government.

3.4. Government Offices. The Offeror shall provide the mailing address, email address, and facility codes for the cognizant Contract Administration Office, Defense Contract Audit Agency (DCAA), and Defense Contract Management Agency (DCMA).

3.5. Teaming Agreements and Identification of Subcontractors. The Offeror shall identify the team structure it will use to meet Government requirements in the Solicitation Attachment 17, Teaming Agreements and Subcontractors. The Offeror shall:

(1) Identify each team member’s company name, address, CAGE code, and UEI number;

(2) Identify the team member’s relationship with the Prime Offeror;

(3) Identify the country(s) in which the prime intends to utilize the team member for performance. If multiple country(s) or region(s) are anticipated, list each in a single cell in Column G, Solicitation Attachment 17; and,

(4) Provide a signed teaming agreement between the Prime Offeror and each team member. The Offeror shall submit a team member or subcontractor consent letter for each team member or subcontractor included in the team structure (see Solicitation Attachment 18 Team or Subcontractor Consent Letter).

3.6. Financial Responsibility Information. To assist the PCO in making the responsibility determination prior to contract award in accordance with FAR 9.1, the Offeror shall provide a written statement explaining the Offeror’s ability to obtain required resources to perform the contract requirements. The Offeror shall also provide evidence to support the explanation. Evidence may include official letter(s) from (a) financial institution(s) demonstrating that the Offeror has the financial resources required to cover all financial commitment. In accordance with FAR 9.104-1(e), the Offeror shall provide an explanation of the Offeror’s organizational controls and measures. At a minimum, the Offeror shall address their accounting controls, operational controls, quality assurance measures, and safety program. The Offeror shall also provide evidence to support the explanation. Evidence may include a list of organizational policies or Standard Operating Procedures (SOPs) which demonstrates the organizational controls and measures of the Offeror. If a list is provided, the Offeror shall include the title of each policy or SOP, the date of each document, and a brief description of each document. The Offeror shall also define which element of FAR 9.104-1(e) the policy or SOP applies to.

3.7. Organizational Conflict of Interest. Offerors shall submit an assessment of potential or actual Organizational Conflict of Interest (OCI) issues, if any, as related to this contract vehicle and future work. Initial notification shall be made to the PCO as soon as they become known. OCI issues include, but may not be limited to, the inability to render impartial assistance or advice to the Government, the inability to objectively perform contract work, or the unfair competitive advantage created by current or previous contractual efforts with the Government. If an actual or potential OCI exists, the Offeror shall submit a mitigation plan with the proposal. The Contracting Officer will review the submitted mitigation plan and make a determination as to whether the plan adequately resolves the issues and provides adequate protection to both the Offeror and the Government. Submission of mitigation plans are only required if the Offeror is aware of potential or actual OCI issues related to this procurement. If the Offeror believes no potential or actual OCI issues exist, this shall be so stated.

3.8. Organizational Structure Change History. Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant past efforts or between conclusion of recent past efforts and this source selection. To facilitate the Government’s evaluation, Offerors shall submit a narrative and/or visual "roadmap" describing all such changes if it is necessary to clarify the contents of the proposals.

4. PROPOSAL VOLUMES

4.1. VOLUME II – PAST TECHNICAL EXPERIENCE

4.1.1. General. The Highest Technically Rated Offerors (HTRO) Minimum Technical Threshold Rating (MTTR) Proposal Volume shall be specific and complete. Legibility, clarity, brevity, and coherence are important. The Offeror’s proposal will be evaluated IAW Section M, paragraph 3, using the Self Scoring Matrix in Solicitation Attachment 12. The Offeror shall only provide relevant pages of documents used to validate Offeror HTRO MTTR Self-Scores. Using the instructions provided below, the Offeror shall provide, as specifically as possible, the actual data used to substantiate the Offeror’s HTRO MTTR Self Score.

4.1.2. Work Sample Cover Sheet. Offerors shall complete a Work Sample Cover Sheet (Solicitation Attachment 15), for each work sample submitted. Offerors shall follow the instructions provided within Solicitation Attachment 15. Each work sample shall provide at least two (2) Government Points of Contact (POC). The Government may contact the Government POCs provided. The Government reserves the right to contact additional Government POCs if necessary.

4.1.3. Work Samples. The Offerors shall submit a maximum of five (5) work samples (contracts/orders) which demonstrate their past technical experience in the required elements. Future performance periods on an existing contract do not demonstrate past technical experience and shall not be utilized as work samples, nor be factored into the total point value of a submitted sample. An acceptable work sample is defined as either a contract, or a task order under a master IDIQ contract (FAR16.501-1), on which the Offeror performed for at least six months with those six months taking place within the last five (5) years of release date of the A&AS V solicitation. A Multiple Award IDIQ or a Single Award IDIQ contract by itself is not an acceptable work sample. An Offeror may choose to use data from up to two (2) Subcontractor contracts/task orders as part of its five (5) work samples, as long as:

1) the Subcontractor was performing as the Prime Contractor on the work sample, AND

2) the Subcontractor is part of the Prime Offeror’s team performing at least 10% of the anticipated effort, AND

3) the Offeror provides signed Teaming Agreements between the Prime Offeror and Subcontractors and completes Solicitation Attachment 18, Team or Subcontractor Consent Letter.

The work samples shall consist of:

1) official contract documentation (including, but not limited to, Performance Work Statements, Statements of Work, Contract Data Requirement Lists [CDRLs] with evidence of the CDRLs acceptance by the Government recipient, Government Furnished Property Lists, Subcontracts, and Invoices);

2) company generated documentation when accompanied by a Statement of Fact (Solicitation Attachment 14) signed by the cognizant Government Contracting Officer or COR to confirm missing information, should contractual documents not carry all of the information necessary to substantiate the Offeror's scores; and/or

3) records from a DCAA-compliant accounting system when accompanied by a Letter of Attestation signed by the individual who has the authority to sign for and legally bind the Offeror. For each work sample, the Offeror shall also include the Government signed cover page associated with the contract or order. Work samples shall include sufficient information to substantiate all points claimed on the Self-Score Matrix (Solicitation Attachment 12).

Work samples shall be provided in their original format. Offerors shall assign each work sample an identifier (e.g., WS1-WS5). All pages of all work samples shall be numbered. In addition, the original pagination of each document shall be retained. If the original documentation within a work sample is not numbered, Offerors shall add page numbers. Font, text size, and margin requirements established in sections 1.9 do not apply to work sample submissions. Work samples shall be in English. If more than one document is provided as part of a work sample, the documents shall be combined as one file and numbered accordingly. Offerors shall include all documentation for a given work sample in a single PDF file, and add to each page a unique WS identifier and page number in the margin (e.g. WS2 pg1). This will ensure that Government evaluators, with the aid of the Cross Reference Matrix (Solicitation Attachment 13), can find all documentation quickly and efficiently, and limit confusion due to multiple page number (page 1 of an SF33, page 1 of the PWS, etc.).

4.1.4. Self-Scoring Matrix. The Offeror shall complete Solicitation Attachment 12 Self-Scoring Matrix. Offerors shall enter their scores in Column J, Offeror’s Self Score, as whole numbers and as hard inputs. Do not enter formulas in Column J. Column K and Rows 8, 14, and 17-23 will auto-calculate. Offerors SHALL NOT alter the formulas in the spreadsheet. If errors are discovered in the spreadsheet, the Offeror shall contact the Contracting Officer. Self-rated scores shall be verifiable using their associated work samples.

4.1.5. Cross-Reference Matrix. Offerors shall enter information that links their self-scores to the substantiating information included in their proposal in the Cross-Reference Matrix, Solicitation Attachment 13. The Cross-Reference Matrix includes a sample response in rows 9 to 11 demonstrating which information should be included in which column. The Offeror shall identify the type of document provided as a work sample in Column F. The Government has included a list of sample documents within the Note in Cell D7; the Offeror is not limited to this list of documents. The Offeror shall include a work sample identifier (e.g., WS1-WS5), document title, document date, page(s), paragraph(s), table number(s) (if applicable), and block(s) (if applicable) within Column F. The Offerors shall identify the contact information for the relevant Government POC in Column G. Multiple references may be included in a single row if:

(1) Multiple references related to the same work sample are necessary to substantiate the Offeror’s claimed points, or;

(2) The Offeror uses the data of a subsidiary or legal entity as a prime contractor work sample. If the Offeror uses the data of a subsidiary or legal entity as a prime contractor work sample, the Offeror shall be sure to include a reference to the objective evidence which demonstrates that they are a legal entity of the parent corporation.

Offerors are encouraged to verify the accuracy of each reference.

The Offeror shall assume that the Government has no prior knowledge of its capabilities and experience. Therefore, the Offeror shall include in column H, any clarifying facts necessary to improve the Government’s understanding and further substantiate the Offeror’s claimed points.

4.2. Evaluation Categories Criteria

4.2.1. Categories 1-7: Recruitment

Category 1: Average time to fill an OCONUS vacancy Of the work samples submitted, the Offeror shall rate itself based on the average time, in days, that it took for the Offeror to fill an OCONUS position vacancy as required by the provided work samples’ PWS. If the Government directed the Offeror not to fill a certain vacancy, this should be excluded from the calculations for average time to fill a vacancy. The Offeror shall provide a Statement of Facts (Solicitation Attachment 14) for any vacancy, required by the work sample, not included in the calculations for average time to fill a vacancy.

Category 2: Percentage of employees with a TS Clearance or higher Of the work samples submitted, the Offeror shall rate itself based on the overall percentage of employees, including subcontractors, that required a Tier 5 (Top Secret) clearance investigation.

Category 3: Fill Rate Of the work samples submitted, the Offeror shall rate itself based on its average fill rate over the previous five years. “Fill Rate” is defined as the percentage of Full Time Equivalents (FTEs)/requirements filled out of the overall required amount of the Offerors requirements to provide A&AS support. The Offeror can include in its count any subcontractor FTEs that were part of the team, but the Offeror had to have been the prime contractor on the effort. If the position incurred a Government-driven delay, the Offeror shall provide a Statement of Fact (Solicitation Attachment 14) and exclude this position from the denominator when calculating the overall percentage.

Category 4: Percentage of SOFA approved employees filling DOCPER-approved positions Of the work samples submitted, the Offeror shall rate itself based on the number of SOFA-approved employees filling Department of Defense Contractor Personnel Office (DOCPER)- approved positions. The Offeror can include in its count any subcontractor FTEs that were part of the team, but the Offeror had to have been the prime contractor on the effort. In order to further substantiate this category, Offerors shall provide confirmation of the SOFA status approval of the employees they are submitting as part of their self-scored rating. This information is ideally provided from the Offeror’s corporate records (e.g., a copy of the DD 1172s showing the contract number and SOFA status, with appropriate redactions) but may be substantiated with information from the Government COR, if necessary. Offerors shall not contact the DOCPER office to request documentation.

Category 5: Number of Work Samples that include support for at least two of the following functional areas: Program Management, ISR support, or Flying Operations Of the work samples submitted, the Offeror shall rate itself based on the number of work samples that included support for DoD, or similar, Program Management, Intelligence Surveillance and Reconnaissance (ISR) support, or Flying Operations. In order to receive maximum points for this category, Offerors must demonstrate that each of their five (5) work samples included at least two of the following functional areas:

Program Management support may include: gathering, analyzing, and measuring the effectiveness, efficiency, and productivity of acquisition programs; assisting the Government in assessing what programs are within schedule and cost baselines, recommending viable solutions to problems and the pursuit of alternative courses of action; providing quality acquisition and program documentation (briefings, documents, plans, etc.); or formulating plans and recommending effective strategies for meeting cost, schedule, and performance objectives.

ISR support may include: providing intelligence data analysis; architecting theater tasking of sensors; exploitation of data; dissemination of intelligence; and network and system administration support for intelligence servers and nodes.

Flying Operations may include, but is not limited to supporting fighter, mobility and rescue flying squadrons in the following areas: scheduling; operations training support; standardization/evaluation support; weapons duty officer support; unit deployment management support; weapons and tactics support; defense readiness reporting system support; and OPLAN/Exercise Logistics support.

4.2.2. Categories 6-9: Retention

Category 6: Number of key corporate personnel/program managers in theater (local presence) Of the work samples submitted, the Offeror shall rate itself based on the number of key corporate personnel it has placed in the local OCONUS area. Key corporate personnel are defined as those personnel who perform a managerial/administrative role and/or provide human resources management functions in support of the company’s management of A&AS contracts. Program Managers are those who act as focal points for employee/customer relations in theater. These personnel are NOT actual positions filled to meet Mission Partner PWS requirements. The Offeror shall only count personnel who were in the same time zone or plus or minus two time zones, as the work directly performed for the contract for 75% or more of the period of performance, or adjusted core duty hours CONUS to align with OCONUS duty location hours.

Category 7: Number of full-time Ordinarily Resident/Other employees Of the work samples submitted, the Offeror shall rate itself based on the number of full-time Ordinarily Resident or other employees hired/retained during the contract period of performance. Other employees includes those that fall into the Appointment Eligible Family Members and Eligible Family Members employment categories as defined by the US Embassy. https://ua.usembassy.gov/wp-content/uploads/sites/151/Definitions.pdf Category 8: Retention Rate Of the work samples submitted, the Offeror shall rate itself based on their average retention rate. “Retention Rate” is defined, in terms of a percentage, to express the number of FTEs/requirements filled out of the Offerors overall amount at the start of the period of performance, and remained in that position throughout the period of performance. This is the inverse of a “turnover rate” which is defined as the number (percentage) of employees who left the company and required the offeror to backfill a position with a new candidate. The Offeror can include in its count any subcontractor FTEs that were part of the team, but the Offeror had to have been the prime contractor on the effort.

The Offeror shall calculate its retention rate as follows: first, identify the total number of positions across all work samples (i.e. the number of positions proposed/required to complete work defined in the PWS), known as “starting headcount”. Then, identify those positions that once filled did NOT turnover during the period of performance, known as “ending headcount”; the Offeror will then divide the ending headcount by the starting headcount and multiply by 100. Offerors will then calculate their average retention rate for their provided work samples. Offerors shall include a worksheet in Excel format showing the calculation with the formulas visible.

Category 9: Number of Work Samples supporting Geographically Separated Operating Sites Of the work samples submitted, the Offeror shall rate itself based on the number of Contracts/TOs where service was performed in at least three (3) geographically separated locations, either CONUS or OCONUS. Geographically separated operating sites must be at least 50 miles apart.

4.2.3 Category 10: Contract Transitions

Category 10: Percentage of FTEs transitioned OCONUS to support a DoD, or similar, service contract w/in Phase-in Period Of the work samples submitted, the Offeror shall rate itself based on the percentage of FTEs transitioned and/or hired within the allotted work sample Phase-in Period. The Offeror can include in its count any subcontractor FTEs that were part of the team, but the Offeror had to have been the prime contractor on the effort. Sole-source contract extensions and bridge contracts for which the Offeror was the incumbent are not considered ‘transitions.’

5. CONTRACT DOCUMENTATION

5.1. Organization. This volume shall be organized according to Table 1 above.

5.2. IDIQ Solicitation/Contract Form. The Offeror shall complete blocks 12 through 18 on the Standard Form (SF) 33 – Solicitation, Offer and Award, for the IDIQ and the same information on the DD Form 1155 for the Initial Task Order. Block 12 shall be filled in with the proposal validity date. Signature by the Offeror on the SF33 constitutes an offer, which the Government may accept. The Offeror shall complete blocks 12, 15A, 15B, 15C, 16, and signature and date for blocks 17 and 18 of the SF 33 along with all certifications requiring signature. The Offeror must be registered in the System for Award Management (SAM.gov) and the Offeror’s legal name and address in Block 15A on the SF 33 must match the information for the Offeror in SAM.gov. The name, title, signature and date identified in Block 16, 17, and 18 must be an authorized representative with authority to commit the Offeror to contractual obligations.

5.3. Provisions and Clauses Fill-ins. Applicable fill-in provisions and clauses shall be incorporated in Volume I. The Offeror shall comply with the provisions within Section K of the solicitation for the IDIQ contract and shall fill in all provisions. If any are not applicable, the Offeror shall annotate N/A so that none of the provision fill-ins are left blank. The Offeror shall confirm that all Representations, and Certifications remain current throughout the evaluation process.

5.4. Section K. Representations, Certifications, and other Statements of Offerors. The Offeror shall comply with the provisions within Section K of the solicitation for the IDIQ contract. The Offeror shall fill in all provisions. If any are not applicable, the Offeror shall annotate N/A so that none of the provision fill-ins are left blank. The Offeror shall confirm that all Representations, and Certifications remain current throughout the evaluation process.

5.5. DD254. The Offeror shall input all Prime Contractor information in the DD254 and submit the document along with the proposal.

5.6. Standard Form (SF) 30. In the event this RFP is amended, the Offeror shall acknowledge all amendments and submit signed copy(ies) of all SF30s as part of this volume.
