About this file
This is a custodial service evaluation checklist issued by the State of Maine Department of Defense, Veterans and Emergency Management - Military Bureau for use at designated military facilities including FMS#1 Saco, FMS#2 Auburn, FMS#3 Bangor, FMS#5 Caribou, Northern Maine Readiness Center (NMRC), Brewer Armory, Lewiston AFRC, Norway Armory, Sanford Armory, Skowhegan Armory, Westbrook Armory, and Camp Keyes buildings. The document establishes performance evaluation standards and procedures for custodial contractors covering daily/weekly tasks (trash removal, recycling, carpet vacuuming, floor mopping, restroom sanitation, drinking fountain cleaning, equipment sanitization), monthly tasks (refrigerator wiping, microwave cleaning, light fixture maintenance, baseboard washing, shower curtain cleaning), quarterly deep cleaning, semi-annual tile and carpet cleaning, and annual floor stripping, sealing, and refinishing. Contractors must conduct work during standard facility hours of 7:30 a.m. to 4:00 p.m., excluding state and federal holidays, and must employ federally vetted personnel. Mandatory performance evaluation tours are scheduled monthly at minimum, with evaluations using a four-tier rating system (Excellent, Good/Meets Expectations, Needs Improvement, and Unacceptable). Contract performance standards define detailed cleaning specifications for various surfaces including plumbing fixtures, dusting, sweeping, spot cleaning, damp mopping, metal cleaning, glass cleaning, and floor finishing.
Poor performance is documented when a contractor receives four or more negative responses to yes/no questions or Needs Improvement ratings during evaluation tours, triggering a two-week correction period; failure to address issues results in contract termination with thirty days' notice. After three documented poor performance evaluations, the contract may be terminated immediately. The contractor has fifteen calendar days to appeal any termination decision to the Director of the Division of Procurement Services. Contractors must supply commercial-grade equipment including HEPA-filter vacuums, carpet shampooing equipment with minimum 50 PSI delivery and 100 inches of water lift, and materials meeting specific recycled content requirements (toilet paper minimum 20% post-consumer fiber, paper towels minimum 40%, trash bags minimum 10%). All products must be PFAS-free with Safety Data Sheets available for review. Completed evaluation forms must be submitted to Kenneth R. Merrifield at kenneth.r.merrifield.nfg@army.mil, with all facility contacts, Department of Finance and Expenditures representatives, and vendor/provider contacts required to sign and date the evaluation documentation.

· Clean up spills as fast as possible. Less time on the floor means less chance of staining.
· Pick up large debris by hand.
· Sweep or dust-mop the entire floor (dirt left on floor will abrade and dull the surface).
· Auto-scrubbing with LS Klean, which contains lithium silicate, helps the pads clean the floor.
· Auto-scrub entire floor.
a. Dilute the cleaners as recommended with clean water in an auto scrubber with light head pressure.
b. Choose a soft cleaning pad based on –pad manufacturer recommendations.
