About this file
Technical Exhibit 6 Forms is a document containing six standardized forms and templates for contract administration and performance tracking for a U.S. Coast Guard facilities maintenance services contract. The forms include a Contract Discrepancy Report, Performance Evaluation Meeting Record, Sample Work Order, Facilities Engineering Performance Measurements Report, Generator Test Report, and Vehicle Dispatch Log. These forms are designed to systematically document work activities, track performance metrics, report maintenance actions, and maintain administrative records throughout the contract's execution.
The associated federal contract opportunity is a Service-Disabled Veteran Owned Small Business (SDVOSB) set-aside solicitation for facilities maintenance services at the U.S. Coast Guard Aviation Training Center in Mobile, AL. The contract requires comprehensive maintenance services including food service equipment, transportation, electrical systems, heating systems, water and sewage systems, air conditioning, building maintenance, grounds keeping, trash disposal, and fire protection services. Key dates include an optional site visit on 1 July 2025, questions due by 2 July 2025, and quote submissions due by 11 July 2025 at 1:00 PM ET. Quotes must be submitted via email to the Contract Specialist and Contracting Officer.
