About this file

This is a Sales and Use Tax Construction Contract Exemption Certificate issued by the State of Ohio for the City of Toledo's Danny Thomas Park Ballfield Sink Hole Investigation and Turf Repair project. The certificate is a pre-filled form (STEC CC Rev. 05/25) designed to exempt tangible personal property and construction materials from Ohio sales and use tax when incorporated into qualifying real property projects. The document identifies the contractee as the City of Toledo, with the project located within the city limits, and requires signatures from the owner/contractee, political subdivision representative, prime contractor, and any subcontractors involved in the work. The certificate must be retained by the prime contractor with copies maintained by the owner and all subcontractors, and signed copies must be provided to suppliers when purchasing materials under the exemption.

The exemption certificate applies to construction projects meeting specific criteria under Ohio Revised Code, including real property owned or to be accepted for ownership by the state of Ohio or its political subdivisions, such as the City of Toledo. The form was executed on January 1, 2026, with the City of Toledo's Commissioner of Purchases and Supplies signing on behalf of the political subdivision at One Government Center, Suite 2000, Toledo, Ohio 43604. Bidders responding to this project must submit the Tax Exemption Certificate along with required paperwork packets including vendor registration, business tax registration, certificates of insurance, worker's compensation documentation, and a bid guaranty to be considered for award by the City of Toledo.
