About this file
This is a Supplemental Bidder Attestation form issued by the State of Rhode Island Division of Purchases under R.I. Gen. Laws 37-2-13.1(b). The form requires bidders and their parent corporations, subsidiaries, affiliates, and subcontractors to certify that they have no conflicts of interest as defined in Chapter 14 of Title 36 with any official, officer, or agency involved in the Request for Proposal. Bidders must also attest that they have not materially participated in or been consulted regarding the requirements, technical aspects, or formation of the RFP. Additionally, the attestation confirms that the RFP does not relate to any audit, examination, independent verification, review, or evaluation of the bidder's work, financials, or operations performed on behalf of the State of Rhode Island or any state official, officer, or agency.
The form requires an authorized signatory to complete the attestation by providing the vendor or entity name, printed name, signature, date, and the solicitation number and title. The document clarifies that being a current or past vendor to the State does not automatically constitute a conflict of interest requiring preclusion from participation. Furthermore, formal responses to a Request for Information issued by the Division of Purchases do not constitute participation or consultation in a subsequent RFP and do not disqualify vendors from future RFP participation.

1 A “conflict of interest” occurs when private interests or relationships interfere in any way with the interests of the
State. Simply being a current vendor or a past vendor of the State does not in itself cause a conflict requiring preclusion. Additionally, formal responses to a Request for Information (RFI) issued by the Division of Purchases does not constitute “participation” or “consultation” with respect to a future RFP and does not disqualify a vendor from future participation from a subsequent RFP.
