About this file
The document is a Subcontractors List form for a bidding process, intended to be completed by contractors participating in the City of Carmel's 2026 Rock Salt for Drinking Water Softening procurement opportunity. The form provides instructions for bidders to list subcontractors who will perform work constituting 5% or more of the total bid amount, with requirements that only one subcontractor be listed per work item and that named subcontractors will be employed upon contract award unless specifically authorized changes are made by the owner.
The form emphasizes that omitting subcontractor names implies the bidder will use their own workforce for most work, and failure to provide complete information may result in the bid being deemed non-responsive. The document provides a structured table with columns for Subcontractor Name, Work, and Price, with a note that additional sheets can be used if necessary. The instructions are designed to ensure transparency in subcontractor selection and prevent bidders from gaining unfair advantages through incomplete disclosure of their proposed work teams.

SUBCONTRACTORS LIST
Instructions To Bidders:
The Bidder shall enter the names and the type of work to be done in the Subcontractors List which follows for each Subcontractor that the Bidder proposes to use for the Work who will be providing work for an agreed price of five percent (5%) or greater of the total Bid amount in the Pricing Table.
Only one Subcontractor shall be listed for each work item. Upon award of a contract, the named Subcontractors shall be employed to perform the work, unless changes are specifically authorized by the OWNER. Failure to furnish all information requested may render the Bid non-responsive if it is determined that such omission is material by affording the Bidder a substantial advantage over other Bidders.
Except as otherwise specifically stated by the Bidder in this Part, omission of any names of Subcontractors herein shall constitute an affirmative representation and statement that the Bidder proposes to use his own work forces for most of the Work such that he will have no Subcontractors providing work in the amount of five percent (5%) or greater of the amount Bid.
