About this file

This is a Subcontractor Summary Worksheet template issued by the City of St. Petersburg, Florida, for use in bidding on municipal construction and service projects. The document establishes the framework for prime contractors to identify and list all subcontractors that will be utilized on awarded projects. Prime contractors must complete and submit this worksheet with their bid, disclosing the names, business addresses, contact information, and trade classifications of all intended subcontractors, along with subcontract amounts. The worksheet specifies that subcontractors are limited to first and second tier only unless otherwise noted in contract documents, and establishes a maximum percentage of contract work that may be subcontracted, which varies by project.

The worksheet prohibits changes or substitution of listed subcontractors after project award and pre-construction meeting, except under specific circumstances including failure to execute contracts, lack of proper licensing, insolvency, ineligibility, performance failure, unsatisfactory work quality, or substantial project delays. Small Business Enterprise (SBE) subcontractors used to meet SBE percentage requirements must be replaced with other SBE subcontractors if substitution becomes necessary. Any requests for subcontractor changes must be submitted in writing to the City's Procurement & Supply Management department. The document notes that subcontracting restrictions do not apply to FDOT LAP or federally funded projects, and includes a legend of reason codes for documenting additions and deletions to the subcontractor list.
