About this file

This is a Subcontractor Summary Spreadsheet from the City of St. Petersburg, Florida, for use in bidding on the Flow Monitor Maintenance Program. The spreadsheet is a mandatory worksheet that prime contractors must complete and submit with their bids. It requires bidders to identify all intended subcontractors, including their legal names, business addresses, contact information, and the specific trade classifications and scope of work each subcontractor will perform. The document specifies that subcontractors may only be changed or substituted after contract award for documented reasons including failure to execute a contract, lack of proper licensing, bankruptcy, ineligibility, performance failures, unsatisfactory work quality, or project delays. Additionally, any Small Business Enterprise (SBE) subcontractors used to meet SBE percentage requirements must be replaced with another SBE subcontractor if substitution becomes necessary, unless the prime contractor demonstrates SBE requirements are otherwise being met.

The spreadsheet requires prime contractors to disclose the percentage of contract work to be subcontracted and to identify whether subcontractors are first or second tier. All subcontractor changes must be submitted in writing to the City's Procurement & Supply Management department with documented justification using the provided reason codes. The form includes fields for tracking additions and deletions with approval dates and signatures from a city approver. Restrictions on subcontracting percentages and multiple-tier subcontractors do not apply to FDOT LAP or federally funded projects. The worksheet must total the subcontract amounts and calculate the allowable subcontracting percentage based on the overall contract amount.
