About this file
This document is a template and instruction set for submitting Certificates of Analysis (COA) and Certificates of Quality (COQ) in response to Defense Logistics Agency (DLA) Energy RFQ SPE605-27-Q-0204, a competitive solicitation for fuel product delivery to support DLA Energy mission requirements.
All certificates must be submitted electronically via email directly from the testing laboratory to the Contracting Officer (Augustus.Patterson@dla.mil, 571-585-1435) and Contract Specialists (Suttom.Beshir@dla.mil, 703-965-7789; Marcus.Nolan@dla.mil, 571-585-9022) by November 18, 2023 at 11:00 a.m. Eastern Time. Offerors are required to notify their testing laboratories of these submission requirements. Acceptable file formats include Adobe PDF, JPEG, JPG, Microsoft Word, Microsoft Excel, and Microsoft PowerPoint. Laboratories and offerors must use the email subject line format "[Solicitation Number] – COA/COQ for [Fuel Product]" (example: SPE605-27-Q-0204 – COA/COQ for SFD). DLA Energy will automatically scan incoming emails for malicious software and restricted keywords; truncated abbreviations or repeated character placeholders should be avoided to prevent filtering delays or quarantine. Offerors must confirm receipt of all transmissions within 48 hours of submission; if confirmation is not received, they must contact the contracting officer or contract specialist immediately. The document emphasizes that certificates must independently satisfy all requirements and quality standards established in the solicitation, and DLA Energy reserves the right to reject any COA or COQ not submitted directly from the testing laboratory. Page 3 provides a blank template form requiring laboratory letterhead and fields for laboratory name, customer name, product type, tank number, batch number, sample number, sample date, date submitted, person who drew the sample, seal date, person in charge of testing, and approval signature.
