About this file

This document contains templates and instructions for recording pest management operations and pesticide use during military deployments. The templates include worksheets for applying pesticides, applying new pesticides not previously recorded, and recording surveillance, non-chemical controls and animal control activities. The pesticides worksheet contains EPA registration numbers and trade names of approved products. The new pesticides worksheet allows recording unlisted products. Instructions specify using separate templates for each location and submitting by the 15th of each month to the appropriate military public health authority based on service branch via encrypted email. Templates must be saved as Macro-enabled Excel files. Guidance is provided on completing each column to document pesticide and non-pesticide applications and meet military recording and reporting requirements.
