SUMMARY OF EXPERIENCE FORM

INSTRUCTIONS: (Page 1 with instructions is not included in the page count)

Instructions: Offerors shall demonstrate a minimum of five (5) years of experience in Ammunition Operations. Additionally, the Offeror shall demonstrate that they have a minimum of five (5) years of experience in seven (7) of the remaining eight (8) functional areas. The prime Offeror must demonstrate the experience in Ammunition Operations, and at least three of the other functional areas. Experience must fall within the past ten (10) years from the date of solicitation release: As part of the submission, the offeror shall include all applicable contract numbers, contract titles, identify the prime or major subcontractor that performed the effort, identify to which ATSS PWS portions the experience applies, and include contract period of performance dates that will allow the Government to successfully evaluate the offeror’s experience volume of the proposal. In order for a contract effort to be considered, the discussion below must confirm that the prime or subcontractor that has the experience identified will support the same competency areas if the offeror receives the resultant award.
