PART TWO – QUESTIONS SUBMITTED VIA EMAIL AND USAID’S RESPONSE

I. SAM and DUNS Registration

Q.18 I have read the posted RFP and viewed all the attached documents. but what remain puzzle is getting DUNS and NCAGE numbers. When the registration is to be active or does it be point of discussion in the premeeting to be held on October 21/2015?

Q.19 While I input my data for DUNS Number XXXXXXXXX the system is responding by notifying me that there is a mandatory error. The exact message is "Although DUNS Number XXXXXXXXX is correctly formatted the information you input did not sufficiently match what is on the file at D&B for this DUNS Number"

I had also tried to search my DUNS Number at fedgov.dnb.com but the search result did not bring my company name.

My guess is now that my company information be it the correct one or the one with character limitation and the G separated by space from ENGINEERING (ENGINEERIN G) is not yet passed to the sam.gov data base.

I have successfully registered on SAM and confirmed my user account. But I would like to get your help on how to register on DUNS.

I have completed SAM registration and received confirmation. As you can see from the entity information below there is a spelling error in Entity name, the word "CONTRACTOR" is misspelled as "CONTRACT OR". I can't remove the space and correct it. Can you tell me what I have to do to make the necessary change in the entity name.

I have difficulties in getting DUNS number. how can i solve it?

Q.20 In due of sam registration process I sussesfully complete the following :

• Finding DUNS number

• Registered/ get NCAGE code

Even more; as per the procedure given in the quick guide for registering in SAM I was able to go through step 1 up to step 5 without any problem.

Yet, under step 6 "complete core data" I could not validate my DUNS information. I have attached the print screen shoot of SAM error message, confirmation from NCAGE code registration together with my DUNS information which has been entered in D&B registration in word file.

I could not figure out why this happen, therefore I am looking forward your support

Mandatory Errors Although DUNS Number XXXXXXXXX is correctly formatted, the information input did not sufficiently match what is on file at D&B for this DUNS Number Although DUNS Number XXXXXXXXX is correctly formatted, the information you input did not sufficiently match what is on file at D&B for this DUNS Number."

Q.21 We wanted to register to express our interest for the construction of the health centers that were announced by USAID. However, we couldn't register .So would you please help us to register.

Q.22 I would like to ask you information about the registration on SAM (System for Award Management) when I have tried to fill the form it rejects my phone number entry I have tried many arrangements but persistently rejected.

Would you please send me the correct arrangement of the phone number to fit the SAM registration format.

Q.23 My company would like to participate on the construction of the health centers, so can you help me with information on pre-proposal conference, site visit schedule, registration in SAM and other requirements that I may fulfill?

Q.24 We want to participate in the pre-proposal conference for national blood transfusion center and 17 health centers, unfortunately the SAM registration system does n' t accept the telephone number and user name as it suggests in the process.

Q.25 I have encounter a problem when i am trying to register for SAM , it keeps on telling me the DUNS no and the information i entered does not much i checked everything a lot of times and i can not pass this stage, what do you think i should do??

Q.26 I want to partiecipate in the construction of the health centers which are posted on October 6 and i just finished registeration on SAM but I didn't know when the site visit takes place, would you mind emailing me?

Q.27 I am writing this emaili getting your address fron news paper for the copy of the re registration document to participate in the bid for construction of buildings to different parts of our company I have a grade 4 general constructor

Q.28 I have finished registration for SAM my DUNS no. XXXXXXXXX and my CAGE no. XXXXX but after congratulations for SAM registration i have received another email cage validation fail because of invalid physical address. please help me to activate registrations

Q.29 We have been recently registered in D&B system and our DUNS number is XXXXXXXXX up on completing DUNS number we request NCAGE code since we are foreign entity not based in US. accordingly we have one XXXXX the next step we did was registering in SAM. we completed the registration process and had a congratulation message yesterday October 26, 2015. yet same day Oct 26 at 10:49 AM we received an other email from samadmin@sam.gov which was sent by an automated administrator saying:

This email is to inform you that the entity registration you submitted to the System for Award Management (SAM) is not able to be processed. This may be due to a problem with the assignment of (or update to) your Commercial and Government Entity (CAG)E Code. The following validation failure reason has been provided to SAM: Invalid physical address does not match NCB data."

in first place since we are a foreign entity i don't need CAGE code and second the physical address part i copy and past all the information about our address. there is a question " physical line address 1" and " physical address line 2" in SAM which is not found in same format in DUNS and am afraid may be i do not know which goes to what.

further i update my DUNS info today in case there is miss mach with sam data, and i find the postal code can not be left empty in DUNS while i leave these question empty in SAM registration and also the phone number entering formats are not identical (SAM's and DUN's format).

we are really looking forward to have your support !

A.18-29

We confirm that the Quick Start Guides (i. Quick Start Guide for Entities Interested in Being Eligible for Government Contracts; ii. Quick Start Guide For New Foreign Registrations; iii.

Quick Start Guide for Updating/Renewing an Entity; and iv. Quick Start Guide For Screen Readers in SAM) made available upon request to potential offerors are perfect tools to ensure successful registration if all the steps outlined in the guides are carefully applied. Potential bidders are advised to ensure a careful application of the steps and the inputting of the required information in consistent manner.

In the meantime, we will communicate to and follow up with the appropriate System Help Desks in Washington to seek additional support in order to resolve outstanding SAM and DUNS registration issues.

Q.54 I have come across your office posting on the construction of 18 health facilities and I have downloaded the RFP. Your office announced that the RFP would be relseased on teh 16th October, but it was delayed till the 20th.

Therefore, I would like to raise the following issues for clarification.

a. Since there was a delay, will there be a rescheduling of the site visit programs?

With http://www.sam.gov/ http://fedgov.dnb.com/webform http://www.sam.gov/ http://fedgov.dnb.com/webform such a design we would take full responsibility for the foundation design, the building design and would be able to save, I believe, a significant amount of time and money for USAID. Please let me know as soon as you have time if this would be a consideration.

A.56 See the Solicitation and the entire package provided in the link above. There is no consideration for a Pre-Engineered Metal Building.

Q.58 We need clarity on the following issues before submitting our proposal.

a) section L.9 of the RFP on page 75 says < Offerors are not required to submit a proposal for all seven projects.> does it means that we are capable of participating in multiple clins with one technical proposal? it is critical question for us because we want to participate on multiple clins and we also made the mandatory site visit in many of the clins.

A.a Refer to USAID’s response to Question No. 14.

b) page 76 under key personnel requirement says ( do not submit copies of degrees, certificates, or license as part of the technical proposal. ) So what documents for the experience of key personnel is to be attached.

A.b See Attachment J.15 incorporated through this Amendment, Amendment 000002.

c) for corporate experience and past performance of the proposal, what is the required attachment for submittal.

A.c See Attachment J.11 and J.12 of the Solicitation respectively.

Q.59 Give me information on pre-proposal conference.

A.59 Refer to Amendment 000001 to the Solicitation.

Q.60 We were really interested to participate in the above refernced tender. Unfortunately we were unable to attend the site visit on the date specified on the RFP. More over On section L it states that the site visit is mandatory however on section M it doesn't have any weighted value.

Hence this is to request your good office that shall we proceed with the tender document preparation or will missing site visit lead to automatic disqualification?

A.60 Please follow the instruction in the Solicitation and the entire package provided in the link above.

The purpose of the mandatory site visits is for exchange of information to improve the understanding of Government requirements and industry capabilities, thereby allowing potential offerors to judge whether or how they can satisfy the Government’s requirements, and enhancing the Government’s ability to obtain quality supplies and services, including construction, at reasonable prices, and increase efficiency in proposal preparation, proposal evaluation, negotiation, and contract award.

Q.61 I want to partiecipate in the construction of the health centers which are posted on October 6 and i just finished registeration on SAM but I didn't know when the site visit takes place, would you mind emailing me?

A.61 Please follow the instruction in the Solicitation and the entire package provided in the link above.

Please note that the solicitation was posted on October 16, 2015.

Q.62 I ask you your good office how to collect the documents and other thing we have to do further.

we already registered in US Government's System for Award Management (SAM).

A.62 Please follow the instruction in the Solicitation and the entire package provided in the link above.

Q.63 This is to remind you to upload us the "BIO form" if i recall the naming correctly.

(a form used to fill the details about newly proposed key personnel )

A.63 See Attachment J.15 incorporated through this Amendment, Amendment 000002.

Q.64 As per your schedule for site visit of health centers, kulmesk, Girana and Gelsha, we have attended in kulmesk site visit.

But we have been told that we could have been attended in all three sites.

We assume that we can participate only in 1 site. If we are supposed to participate in all three projects tender, we are capable and have enough information about all 3 sites, even though our best interest is in Kulmesk as we have familiar to local conditions

Hence kindly request your good office to conform us if we are able to participate these tenders so that we can prepare our tender before due date.

We appreciate your consideration and your immediate response

A.64 Please follow the instruction in the Solicitation and the entire package provided in the link above.

Also, refer to USAID’s response to Question No. 14.

Q.65 I was fulfilling all requirements to give my proposal for SOL-663-16-000001.

But due to many reasons I didn't attend the site visit.

This is therefore to request you if any chance of visiting the site i preferred to participate.

A.65 USAID doesn't have a plan to re-schedule the site visits.

Q.66 We intend to bid for the above mentioned project. We have participated in all the mandatory site visits expect the group 7 (site 7.1 and 7.2) as the vehicle was broke down on the way to project site. We would like to know if we could submit our proposal for all the groups as one proposal or each group's proposed proposal should be sent separately. Please clarify !

A.66 Please follow the instruction in the Solicitation and the entire package provided in the link above.

Q.67 Our construction Firm has participated on the site visit of Gelsha, Girana, and Kulmesk. However due to misunderstanding of the route, and late arrival, we couldn't be able to register on time for Gelsha site. All was in one group and later due to the feeling that we can participate on at least sites we visited only, we started the preparation of the tender. However there is a remour that contractors we hadn't visited all the sites in a group, will not be allowed to participate on the tender.

Therefore we want to participate on this tender. We have visited all the sites. We registered for the two sites, but we were late on Gelsha site.

A.67 Please follow the instruction in the Solicitation and the entire package provided in the link above.

Q.68 I have the following questions.

a. We did n’t receive confirmation of our site visit till now, shall we except to receive it is not necessary?

A.a Please follow the instruction in the Solicitation and the entire package provided in the link above. Also, refer to USAID’s response to Question No. 14.

b. Regarding the key personnel requirement, page 76 of the solicitation says, “Do not submit copies of degrees, certificates, or licenses as part of the Technical Proposal. Offerors must propose only one set of Key Personnel (one COP/CPM, one Construction Superintendent, and one Construction Quality Manager) in the proposal, regardless of the number of Project Groups.”

For project Group 3, there are three sites. According to the above statements, we have to submit one COP, one Construction Superintendent, and one Construction Quality Manager for all the sites, is that true? And are we going to submit only CV of three employees?

What types of evidences are going to provide for these professionals?

On the other hand, page 16 says, “Each Project Group must have its own Project Manager. Each project site must have its own Superintendent and Quality Manager. For example, for Group 1, the Contractor must have one Construction Project Manager (one for the Group overall), two Construction Superintendents (one for each project site), and two Construction Quality Managers (one for each project site).”

We are confused by these sentences, can you please elaborate?

A.b One COP is required regardless of the number of groups; however, each Project Group must have its own Construction Project Manager. Each project site must have its own Construction Superintendent and Construction Quality Manager. For example, if an offorer is awarded three groups each having three sites; it is required to have a COP, three Construction Project Managers, nine Construction Superintendents and nine Construction Quality Managers.

Q.69 I have visited kulmesk & Girana site also Dansha but we were late to be on Dansha so we are not registered on the list at Dansha we apologize for not be there on time anyway we visited three of them so we would like to participate in this bid.

A.69 Please follow the instruction in the Solicitation and the entire package provided in the link above.

Q.70 XYZ is a USAID PVO specializing in the equipping of hospitals and health facilities. We have a substantial XXX grant, and are approved to use this (by the Ethiopian Mission USAID) for XXX Hospital. XYZ has also worked very successfully with XXX to provide XX maternal health clinics with safe birthing equipment in XXX as part of the USAID XXX program.

XYZ would like to be part of this effort to construct and equip the Blood Transfusion Center and the 17 Health Centers. I am writing at this stage because we have found that reviewing and advising the architectural and construction team prior to building completion can save hundreds of thousands of dollars in either retrofitting, construction remodeling, or extra equipment costs, by ensuring that the actual facility is able to receive and properly install/manage the necessary equipment.

What is the process for XYZ to participate in this effort? Please let me know how best to submit a proposal, or respond to a request, as we are in the right position to help with this project.

A.70 Please follow the instruction in the Solicitation and the entire package provided in the link above.
