About this file

This is a Subcontractor Summary Spreadsheet worksheet issued by the City of St. Petersburg, Florida, for use in bidding on a Shelter project. The document serves as a mandatory submission requirement for all bid proposals and outlines the framework for identifying and managing subcontractors throughout the project execution. Prime contractors must complete the worksheet by entering their company name, the completing employee's information, and detailed data for each intended subcontractor, including the subcontractor's legal name, primary business address, project supervision contact information, trade classifications, and subcontract amounts. The worksheet establishes that up to 80 percent of the contract work may be subcontracted for the Shelter Bid project.

The document establishes strict controls on subcontractor changes and substitutions, permitting modifications only under specified circumstances such as licensing deficiencies, insolvency, substandard work performance, or substantial project delays. Small Business Enterprise (SBE) and Minority/Women Business Enterprise (MWBE) subcontractors used to meet percentage requirements must be replaced with qualified SBE subcontractors if substituted, unless the prime contractor demonstrates compliance with SBE requirements through alternative means. All requests for subcontractor changes must be submitted in writing to the City's Procurement & Supply Management department, specifically to the Contracts Compliance Manager. The worksheet includes columns for tracking added or deleted subcontractors with corresponding reason codes and city approver documentation. The instructions note that restrictions on subcontracting percentage and multi-tier subcontractor use do not apply to FDOT LAP or federally funded projects.
