Section J, Attachment J.8 – EAP and WL Sample Deliverable Schedule
SAMPLE REPORT(S)/DELIVERABLES AND DELIVERY SCHEDULE
The contractor shall submit all required report(s)/deliverables in accordance with the following schedule:
All reports shall reference and cite the contract number. The dues dates listed are in working days, unless otherwise specified. Deliverables shall be sent by electronic mail to the COR, unless otherwise specified.
Any deliverable or products produced under this contract shall be accepted or rejected in writing by the COR.
DELIVERABLES
SECTION DELIVERABLES DUE DATE
Contract Phase-in Services (C.4.1)
The Contractor shall submit the final Transition Plan that details the methods that will be used to ensure a smooth transition from the incumbent Contractor‘s operation to sole operation by the Contractor
Within fifteen (15) calendar days of award
Staff Credentialing (C.5.3)
The Contractor shall submit Staffing Reports that include staff names, addresses, positions, supervisors, training, continuing education, CEAP certification, and security clearance status
Quarterly, within three (3) business days of the end of each quarter
Network Consideration (C.8.6.)
Contractor shall submit a monthly case management report
Within five (5) business days after the end of each month
Ownership and Destruction (C.15.9 & C.16.)
The Contractor shall submit all electronic and print records to FOH as requested.
Within three (3) business days of the COR’s request
Incident Reports (C.9.2)
The Contractor shall submit an Incident Report that summarizes low profile crisis incidents that occurred during the previous week.
Close of business each Monday
Incident Reports (C.9.2)
The contractor shall submit high-profile incident reports (initial incident report, post-incident report, narrative post-incident report, customized incident reports for federal disasters).
Within twenty-four (24) hours of any high profile incident; within five (5) calendar days of the conclusion of the high-profile incident; within three
(3) business days of the COR’s request for a narrative post-incident report, within three (3) days of a request of the COR for customized reports in response to federal disasters.

Program Promotion (C.11.3)
The Contractor shall submit an initial promotional campaign calendar
Within thirty (30) calendar days of contract award
Program Promotion (C.11.3)
The Contractor shall submit an annual promotional campaign calendar
Within ninety (90) calendar days of the end of each fiscal year
Program Promotion (C.11.3 & Sec. J – Attach. 4)
The Contractor shall submit monthly campaign content
In accordance with the timeline established in Section J – Attachment 4.
CQI Program Plan (C.19.B.2.)
The Contractor shall submit a CQI Program Plan
Within thirty (30) calendar days of contract award
Semi-Annual CQI Program Plan (C.13.2)
The Contractor shall submit an semi-annual CQI Work Plan
Within thirty (30) calendar days of the end of each fiscal year and within 30 calendar days of the end of Quarter 2.
CQI Reports (C.13.3.)
The Contractor shall submit an annual, quarterly and monthly Continuous Quality Improvement (CQI) Report
Annual – within fifteen (15) calendar days of the end of each fiscal year Quarterly – within fifteen
(15) calendar days of the end of each quarter Monthly – within five (5) business days of the end of each month
Program Promotions (C.11.3) The Contractor shall provide an initial list of topic areas for the website
Within thirty (30) calendar days of the contract award

Within fifteen (15) calendar days of the end of each fiscal year
CQI Reports (C.13.4)
The Contractor shall generate and provide Customized ad-hoc reports
Within three (3) business days of the COR’s request
508 Compliance (C.17)
The government reserves the right to perform testing on required ICT items to validate the contractor’s Section 508 conformance claims.
The Contractor shall respond to and remediate any problems that are discovered as a result of the testing.
