About this file

This document is a template form for offerors to identify and disclose local area concerns within a half-mile radius of a proposed site for a Remedial Response Contract (RRC). The form requires contractors to complete a table listing all nearby facilities that could potentially raise public concern, including schools, day-care centers, historical landmarks, and other residential facilities. There is no predetermined limit on the number of entries.

The form contains three columns: Business Name & Address, Distance From Proposed Site, and Offeror's Narrative Concerning Possible Opposition Near Proposed Site. Offerors must provide their name and address of the proposed site along with the associated RFP number at the top of each table. The template provides multiple blank tables to accommodate additional entries as needed. This disclosure requirement ensures that contractors proactively identify and address potential community concerns related to the proximity of sensitive facilities to the proposed RRC location.
