About this file
This is a Service Contract Business Management Questionnaire template issued by the Federal Bureau of Prisons (BOP) for evaluating contractor qualifications and performance capability.
The questionnaire is submitted when required by a solicitation, upon Contracting Officer request, or by public notice authorization. It is designed to assess organizational qualifications, responsibility, experience, financial capability, and past performance to determine if an organization warrants further evaluation or contract consideration. Respondents must provide current, accurate, and complete information organized into several key sections: instructions for filing items 1-5 (solicitation identification, legal organization details, principal points of contact, office location, personnel counts by discipline, subcontractor and team member identification, and five most recent relevant contracts); organizational information including business structure, incorporation/partnership details, licensing, and trade name registration; experience documentation including work categories, claims and litigation history, bankruptcy status, and federal contracts performed in the past five years; and financial information including trade and bank references plus proof of financial capability through audited, reviewed, or certified CPA-prepared financial statements (balance sheet, income statement, and statement of cash flows). The questionnaire requires certification under penalty of 18 U.S.C. § 1001 that all information is true, accurate, complete, and not misleading. Respondents must identify no more than two authorized principals to serve as points of contact and provide detailed project summaries for each of the five most recent contracts, including project name and location, services description, contract number, contract type, work complexity level, customer contact information, contract value, status, award and completion dates, subcontracting description, and Contracting Officer's Representative contact information if applicable.
