CONTINUATION PAGE – Amendment Items
The purpose of this amendment is to make the below changes to the cited documents. The previously provided SAQMMA14R0042 RFP, Attachment 1, Attachment 4, Attachment 7, Attachment 12, Exhibit 1, Exhibit 2, Exhibit 4, Exhibit 4a and Exhibit 5 are hereby replaced in entirety with the attached conformed versions. Amendment 1 changes are highlighted in the provided documents.
Additionally, Exhibit 3, Exhibit 3a, and Exhibit 3b are hereby incorporated into the SAQMMA14R0042 solicitation.
SECTION B
NO. REFERENCE CHANGE
1 Section B.4.8, Taxes
Exhibit 1, 7.0, Pricing Information and Instructions
Exhibit 7, TO-001 PEO Tab Task Orders 001-005, Operations CLIN
(e.g. CLIN 002 of TO-001)
Amended to state that taxes are to be included within the FFP rates on each task order.
The CLIN for taxes, within TO-001, has been removed.
SECTION C
1 Section C.2, Background Information Amended to add the following underlined language: “The Contractor shall to take appropriate security precautions, including training of personnel, in order to ensure the safety of personnel, and the continued performance of the contract during the entire period of performance.”
2 Section C.3.1, Kabul The footnote under the Camp Sullivan description has been deleted.
SECTION H
1 Section H.7, Contractor Personnel Medical
Requirements Section H.7 has been amended to add the following language: “U.S. Mission medical unit facilities and clearances are not available to contractor employees, unless approved by the Contracting Officer, U.S. Mission, and regional bureau in writing.”
2 Section H.23.2.1, Task Order Management Plan (TOMP)
Amended to state that letters of commitment does not count against the 30 page limit.
Amended to state that resumes are limited to 2 pages.
3 Section H.30, Pre-Deployment Counter Terrorism Training
The requirement for FACT training, which included defensive driving maneuver training, has been removed.
4 Section H.30, Pre-Deployment Counter Terrorism Training
Amended to clarify that OT401 is required for AN and TCN contractor personnel that are expected to be in country for less than 45 calendar days.
SECTION L
1 Section L.15, General Proposal Instructions Section L.15 now includes the following language: “Additionally, there is a page limit of
24 for the SOW, for task orders requiring one. This allows for a maximum of 54 pages for responses which include the TOMP (30 page limit) and the submission of a SOW (24 page limit).
Within the volume, Offerors shall provide the SOW after the TOMP. Offerors shall provide tabs clearly separating the sections.
TO-002: There is an additional limit of 24 pages each for the Operations Plan and the Transition Management Plan, allowing for a total of 102 pages for TO-002. Within the volume, Offerors shall provide the Operations Plan and Transitions Management Plan following the SOW. Offerors shall provide tabs clearly separating the sections.”
2 Section L.15, General Proposal Instructions The solicitation language has been changed to say “2 pages (1 double sided page)”.
3 Section L.15, General Proposal Instructions Section L.15 has been amended to allow for size 10 point font on tables, graphs, headers, and footers.
4 Section L.15, General Proposal Instructions Section L.15 now includes the “Past Performance Questionnaire (PPQ ) responses” under the excluded sections list of Volume 3.
5 Section L.15, General Proposal Instructions Section L.15 language has been amended to show that the table of contents is excluded from the Volume 6 page count.
6 Section L.15, General Proposal Instructions Section L.15 now states that title pages and any presented glossaries of acronyms and abbreviations are excluded from the page count.
7 Section L.16.2.1.1, Executive Summary Section L.16.2.1.1 has been amended as follows: “The Executive Summary and history is limited to 5 pages and will be an attachment to the summary.”
8 Section L.16.2.1.3.1, Program and Logistics Management
Section L.16.2.1.3.1 language has been amended to include the following statement:
“Copies of contractor held licenses and permits shall be provided as an attachment to the volume, and shall be excluded from the page count.”
9 Section L.16.3.2, Past Performance The solicitation language has been changed to say “…as directed in Attachment 5.”
10 Section L.16.4.2.3.2, Labor Skill Mix-Other The following update has been made: “It is the Government’s intention to issue a task order for Vehicle Maintenance within the base year of this task order.”
Section L.16.5.6.2, Reserved Duplicate language related to licenses and permits was found in Sections L.16.5.6.2 and
L.16.2.1.3.1. The language from Section L.16.5.6.2 has been removed.
ATTACHMENT 1, PDS AND QUALS
1 Attachment 1 Clearance levels for labor categories that previously did not clearly articulate the level have been added.
2 Attachment 1 Fire prevention personnel have been added.
ATTACHMENT 4, SMALL BUSINESS (SB) SUBCONTRACTING PLAN
1 Attachment 4 Note changed to read, “NOTE: The Contractor shall submit an Individual Subcontract
Report, via eSRS, for its domestically owned and operated small business subcontracted work on a semiannual basis, for the periods ending March 31 and September 30.”
ATTACHMENT 7, PRICING TABLES
1 Tab: Summary of Task Orders Cell C4 (value of TO-003) now automatically fills the value based on the total found in cell
C60 of the TO-003 tab.
2 Tab: TO-001, PEO CLIN x004 for Security Evacuation Insurance has been removed. CLIN have been renumbered.
3 Tab: TO-003, Fire Protection Services Tables have been updated based on the A001 presented TO-003.
ATTACHMENT 12, VETTING PACKAGE STATUS UPDATE
1 Attachment 12 Definition of columns provided.
