About this file

The document is a Sample Employment Survey (Form S3P-2) from the Arizona Department of Housing (ADOH) related to a federal housing and community development funding project. The survey is designed to gather potential employment and training information from individuals interested in construction and related work for an upcoming project. The form allows respondents to indicate their current skills, experience levels, and training interests across various job categories including plumbing, carpentry, roofing, painting, landscaping, electrical work, and other construction-related trades. Participants can also specify support services they might need to accept employment, such as child care, transportation, or language assistance.

The survey is part of a broader initiative by a recipient of state housing funds to hire additional staff and contractors for community improvement activities. It provides an opportunity for potential workers to express interest in both current employment and future training opportunities across a wide range of construction and maintenance skills. The form enables the recipient to build a pool of potential workers with varying levels of experience and to understand the support services that might be necessary to facilitate employment for different individuals. By collecting this comprehensive information, the agency can better match potential workers with job opportunities and training programs while addressing potential barriers to employment.
