Statement of Work 676-20-004 Replace Chemistry Analyzer

Tomah VAMC, Tomah, Wisconsin

I. BACKGROUND:

Building 400, Second Floor Chemistry Laboratory currently has an outdated Chemistry Analyzer and will be replaced by a new VA provided Siemens brand Analyzer. Preparation of this space is the objective of this project. Both systems will be located and functional in the same space of the existing lab space during this project and for six months after the installation due to testing and verification. This construction project will remodel the lab and surrounding area to accommodate both systems.

The lab will remain occupied and functional during construction activities. Significant effort will be required to maintain Infection Control Barriers and negative pressure in the construction space due to the extremely high level of air quality requirements. This project will require close coordination with the COR, Laboratory Supervisor, and various lab equipment suppliers during equipment movement phases.

II. SCOPE OF WORK:

The project design was developed by Nagel A/E, Elm Grove, WI. The design was developed in 2021, to include 6 phases of construction. This project shall be completed in accordance with the drawings and specifications, local and state codes, VA Guidebooks and Requirements as presented in the design documents.

The construction will include, but not limited to:

A. Preparing the laboratory to accept the new analyzer system, modifying an AHU in the Building penthouse, and extensive remodel of the second-floor wing as depicted in the drawings.

B. Relocate utilities as required including plumbing, drainage, chilled water piping, sprinkler piping, electrical conduits, fire alarm wiring, IT, lighting, and HVAC ducting that may be affected by the project. Provide TABs for each system affected.

C. Refer to drawings and specifications for more information.

D. Provide Submittal Exchange Services for A/E, GC, and VA prior to submittals for this project.

III. CODES, REGULATIONS, AND STANDARDS:

The following codes and regulations shall be used for the design & construction of this project, whether or not they have been legally adopted by an agency or jurisdiction. Errata, supplements, etc. shall be considered part of the codes. Standards and other publications referenced by the following codes and regulations shall be followed. The codes listed below may not be all inclusive. The A/E shall determine the codes, regulations and agency requirements that apply to this design.

The Department of Veterans Affairs Office of Construction & Facilities Management provides numerous documents pertaining to the construction of facilities for the department. The selected General Contractor shall use the resources provided to minimize errors and/or conflicts in the construction.

IV. ADDITIONAL REQUIREMENTS

A. The contractor will provide and labor, materials, testing, supervision, and equipment necessary to construct and prepare area to accept new analyzer as described in the drawings and specifications.

B. The contractor will be responsible for organizing, scheduling, and facilitating the progress of work.

C. Normal working hours are 7:00 am to 4:30 pm.

D. Provide for the safety of all employees, contractors, visitors, and others involved with the project site.

E. This project will contain ICRA’s and PCRA’s. (See specs and drawings for details).

F. Contractor and sub-contractors will need to provide sanitary facilities for their employees and sub-contractors.

G. Contractors and sub-contractors shall be required to obtain, wear, and return VA

Contractor badges.

H. Contractors and sub-contractors shall wear proper PPE including safety shoes, safety glasses, and hardhat and additional PPE as required by the task being completed.

I. Contractors must comply with daily COVID screenings. Currently this means daily temperature collection, wearing a sticker confirming compliance, wearing a cloth face mask while inside the buildings including the construction area. Additional requirements may be imposed or released over time, during the construction period.

J. Contractor shall be required to comply with the General Requirements, Specification 01 00 00.

K. Work shall be scheduled as materials are approved, available, delivered on site and ready for installation and shall be installed according to the manufacturer’s instructions and according to the specifications. Submittal requirements include:

1. Material Data Sheets (MDS) for all hazardous materials to be used/kept on the contract site.

2. Manufacturer’s literature, hardware, access controls, and installation/operation instructions.

L. All new material stored on site shall be protected from weather until used.

M. The contractor shall provide marked-up drawings and prepare As-Built drawings.

N. Provide detailed CRITICAL PATH construction schedule for minimal interference with

VA operations.

O. Perform and follow up on final inspection. Conduct a detailed inspection including mechanical/electrical spaces, ceiling and chase cavities, and all support areas covered by the contract. Develop punch lists and monitor corrective actions.

P. The work area shall be clean and picked-up at the end of each day’s work.

Q. Contractor will attend bi-weekly progress meetings to discuss accomplishments and immediate future planned work on the project. The contractor shall plan/prepare the agenda and produce minutes.

R. The VA does not anticipate lead or asbestos abatement in the project. However, the contractor shall be cautious and investigate potential asbestos containing materials that may be hidden within walls. Report potential ACM to the COR for further action.

S. The Contractor is to provide a spreadsheet to the COR of all newly installed Building Service Equipment for this project at the end of the contract. The list will include Nomenclature, Location, Model Number, Serial Number, warranty start date, warranty duration, warranty parts dates, and estimated value.

V. TIMELINE (CONTRACT DURATION):

The period for construction activities from the date of the Notice to Proceed (NTP):

T1 = NTP

T1 + 15 days = Pre-construction Kick-Off Meeting, including all sub-contractors T1 + 90 days = 50% complete T1 + 180 days = 100% complete T1 + 195 days = Delivery of marked up drawings for preparation of As-Built drawings and project closeout.

NOTE: days are calendar days.
