About this file

This document is a Responsibility Inquiry attachment for a state procurement opportunity by the State of Oregon, specifically OregonBuys Bid #S-66000-00013333 for Oblique Aerial Photography for Improved Decision Support on the Oregon Shore. The document appears to be a detailed questionnaire designed to evaluate a potential contractor's financial stability, legal standing, performance history, and capability to execute the proposed project.

The form requires potential offerors to disclose critical business information, including their financial resources, past contract performance, legal history, licensing status, and organizational capabilities. Key areas of inquiry include the contractor's ability to meet contractual responsibilities, history of contract completions, potential criminal or civil legal issues, financial health, bankruptcy history, licensing, and compliance with state requirements such as the Pay Equity Certificate (applicable for companies with 50+ full-time workers and contract values exceeding $500,000). The document serves as a comprehensive pre-award assessment mechanism to ensure the selected contractor is responsible, capable, and meets the state's procurement standards.
