About this file
This is a Responsibility Inquiry form (Attachment G) for RFP 6225 issued by an Oregon state agency through the OregonBuys procurement system (Bid # S-44300-00016027) for a Reverse Osmosis Point of Use Water Treatment Installation and Maintenance contract. The form serves as a pre-award evaluation document to determine contractor responsibility and capability prior to contract execution. Proposers must complete the form by providing detailed responses to eight primary inquiry categories and obtain authorization signatures from an authorized company representative. The form does not specify response dates, bidder conferences, site visits, or award dates within its content, as it functions as a post-selection responsibility assessment tool rather than a solicitation announcement document.
The Responsibility Inquiry form requires proposers to demonstrate financial stability, relevant experience, legal compliance, and operational capability across multiple dimensions. Key evaluation areas include availability of appropriate resources and expertise; contract performance history over the past five years regarding budget and schedule adherence; absence of criminal or civil charges within three years; absence of contract defaults or legal disputes; current financial health and cash flow sufficiency; bankruptcy or insolvency history; possession of required licenses and authorization to conduct business in Oregon; and submission of a Pay Equity Certificate if applicable (required for firms with 50+ full-time employees and contracts exceeding $500,000, with specific exemptions for certain professional services). Proposers may be required to submit supporting documentation including profit-and-loss statements, balance sheets, cash flow information, credit reports, bonding capacity information, and performance records from previous contracts.
