About this file
This document is a Questions and Answers Set #1 issued by the State of Oregon, Oregon Health Authority (OHA), Public Health Division, Environmental Public Health Section, Domestic Wells Safety Program, regarding Request for Proposals (RFP) 6225 for Reverse Osmosis Point of Use Water Treatment Installation and Maintenance. The RFP was posted on OregonBuys with Bid Number S-44300-00016027 and has a proposal submission deadline of March 6, 2026, at 5:00 PM Pacific Time. The document addresses four clarification questions submitted by potential proposers and provides official OHA responses to each. The Single Point of Contact for the RFP is John F. Gardner, reachable at 503-509-5698 or john.f.gardner@odhsoha.oregon.gov.
The Q&A document clarifies several key aspects of the RFP: the correct lab analysis fee is $40.50 per test sample (not $37.80 as stated in Section 2.4.2.1.3), with this correction to be formalized in Addendum #1; the Reference Check Form (Attachment D) requires proposers to submit three references with contact information, and OHA will contact references directly without a questionnaire; proposals may be submitted via OregonBuys or email to the SPC; and contractors previously awarded contracts under the prior RFP 6192 are ineligible to submit proposals for RFP 6225. These clarifications will be incorporated into Addendum #1, which was to be posted on OregonBuys shortly after the Q&A issuance date of February 20, 2026.

Question # RFP Section (Page #) Question
1. Section 2.4.2.1.3, item c) and Attachment E - Price Proposal
RFP Section 2.4.2.1.3 item c) currently states:
““Contractor shall ***
c) pay the OHA contracted laboratory for: i) analysis of the test sample; and ii) delivery of the test results to Contractor, the Program Participant, and OHA (via email to domestic.wells@odhsoha.oregon.gov).
As stated in Attachment E – Price Proposal, OHA will pay Contractor a fixed fee of $37.80 for the costs for lab analysis of each test sample.”
But Attachment - E - Price Proposal, says the lab analysis fee amount is $40.50, not $37.80. Please clarify which fee amount is correct - $40.50 or $37.80?
OHA Response to above question:
The correct lab analysis fee amount is $40.50. The lab fee listed in Section 2.4.2.1.3 c) ($37.80) is incorrect and will be changed to $40.50 by Addendum #1 to RFP 6225, which will be mailto:john.f.gardner@odhsoha.oregon.gov mailto:domestic.wells@odhsoha.oregon.gov

2. Section 3.3.3 Reference Check Form (Attachment D) (Page 14), and Attachment D- Reference Check Form
The Attachment D – Reference Check Form contains spaces to complete for three references but includes no questionnaire.
Is there a questionnaire that Proposer references must complete?
Or will Agency (OHA) contact the Proposer references?
OHA Response to above question:
To simplify the Proposal process for this expedited RFP, there is no questionnaire for references to complete, but Proposers must complete the Attachment D- Reference Check form as required by RFP Section 3.3.3. Note: the language of Section 3.3.3 will be revised to add the text in bold underlining below in Addendum #1, to RFP 6225, which will be posted on OregonBuys soon.
“3.3.3 Reference Check Form (Attachment D)
The Proposer shall submit with its Proposal: one Reference Check form (Attachment D) listing the names and contact information of three (3) customers or clients for whom Proposer has provided services similar to those described in Section 2.4 of this RFP within the last 5 years in or around Morrow and/or Umatilla Counties. References must be able to verify the quality of previous, related Work.
Agency may check to determine if references provided support Proposer’s ability and its Key Persons’ ability to comply with the requirements of this RFP. Agency may use references to obtain additional information, or verify any information needed. Agency may contact any reference (submitted or not) to verify Proposer’s qualifications.”
