About this file

This is a Price Proposal Form (Attachment E) for RFP 6225 issued by the Oregon Health Authority (OHA) through OregonBuys for reverse osmosis point-of-use water treatment system installation and maintenance services. The RFP seeks proposals for providing complete water treatment solutions to program participants, with a contract term running approximately from March 15, 2026 through June 30, 2027. Proposers must submit detailed line-item budgets covering all costs associated with treatment installation tasks, including purchase and installation of one NSF/ANSI Standard 58 reverse osmosis system (either a Culligan Aquasential RO System or Kinetico K5 Water Drinking Station), labor for installation including travel time and Spanish language services if applicable, and post-treatment water sample collection and laboratory analysis. The price proposal also requires quotations for maintenance tasks including labor for routine maintenance appointments, product costs for filters and membranes, and post-treatment nitrate testing.

Pricing must be submitted on a fixed-price-per-program-participant basis for both installation and maintenance tasks. The RFP establishes baseline pricing guidance, with OHA-estimated equipment costs of $1,725 for the Kinetico system and $2,350 for the Culligan system, filter and membrane replacement costs estimated at $270 to $300, and laboratory testing fees fixed at $40.50 per test. All labor charges must include travel expenses and travel time to and from participant homes, which will not be reimbursed separately. Proposers are required to calculate and provide total fixed prices for treatment installation tasks, maintenance tasks, and combined tasks per program participant. The form explicitly states that OHA-estimated prices are provided for information purposes only and do not establish contract pricing.
