About this file
This is an RFP Submission Checklist for the City of Philadelphia Professional Services contract opportunities at or above the formal RFP threshold. The checklist serves as a compliance tool to ensure applicants submit complete, accurate, and timely proposals. Key submission requirements include meeting the stated deadline, demonstrating how the applicant meets the Department's Scope of Work requirements detailed in Section II of the RFP, providing a cost proposal consistent with specified requirements, and adhering to overall format and content standards outlined in RFP Section III. Applicants must review the entire RFP and Contract Attachments, including Appendix A (General Provisions), and must disclose any objections to these provisions within their proposal, as objections are not favored and may be weighed against the proposal. All proposals must be submitted electronically through eContractPhilly under the correct opportunity number and signed by an authorized signatory.
Required documentation includes Appendix C (City of Philadelphia Tax and Regulatory Status and Clearance Statement), a Statement of Financial Capacity, and either audited or unaudited financial statements, bankruptcy disclosure for the past five years, or IRS Form 990 for non-profit organizations. Applicants must disclose all information required under Chapter 17-1400 of the Philadelphia Code, including local political contributions, through electronic forms in their eContractPhilly account. Optional submissions include Appendix D (Local Business Entity or Local Impact Certification) and Appendix E (LGBTQ Applicant Opportunity Data). The applicant's registered name and Tax Identification Number (TIN) on eContractPhilly must match the information on the application and W-9 form to ensure proper identification and contract execution.
