About this file
This document is a detailed questionnaire for RFP No. 2130386 issued by the City of Miami for a Group Health Plan Administrative Services Only (ASO) Provider. The questionnaire is designed to comprehensively evaluate potential health plan providers across multiple critical areas including organizational overview, network adequacy, claims administration, member services, compliance, plan design, performance guarantees, and administrative fees. The document strongly encourages complete responses, noting that failure to provide thorough information could negatively impact the proposal's overall evaluation score.
The questionnaire covers extensive ground, requesting information on healthcare services, provider networks, claims processing, member engagement, compliance protocols, and financial terms for the period of 1/1/2025-12/31/2028. Key requirements include detailed submissions on network access fees, telemedicine services, utilization management, case management, nurse line services, disease and maternity management, medical-pharmacy integration, and cost reduction strategies. The City is seeking a wellness credit of $250,000 and a technology credit of $30,000, and requires providers to outline their administrative service fees, performance guarantees, transition processes, and qualifications for servicing a municipal health plan.
