About this file
This document is a City of Miami Questionnaire for Request for Proposal (RFP) No. 2130386 for an Employee Assistance Program (EAP). The questionnaire is designed to gather comprehensive information from potential providers about their EAP services for the City of Miami. The document outlines seven key sections of inquiry: Financial Stability and Experience, Program Design & Services, Access & Coverage, Pricing & Cost Structure, Reporting & Data Access, Compliance & Data Privacy, and Implementation & Support. While completion is not mandatory, the city strongly encourages respondents to fill out the questionnaire, as incomplete responses may negatively impact the proposal evaluation score.
The questionnaire seeks detailed information on potential EAP service providers, including their financial ratings, service delivery methods, counseling session offerings, access protocols, pricing structures, reporting capabilities, data privacy measures, and implementation support. Specific areas of interest include the number of face-to-face counseling sessions, availability of telephonic and virtual counseling, 24/7 program access, family member coverage, billing processes, standard and custom reporting options, compliance with privacy laws, and strategies for employee engagement. The goal appears to be selecting an EAP vendor that can provide comprehensive, confidential, and accessible mental health support services for city employees.
